He was given the Moderna COVID vaccine first dose on 1-15-2021. 2.5 weeks later he complained of constant abdominal pain that lasted two days. No fever/chills, nausea/vomiting, diarrhea/constipation, no visible blood in stool or urine, no abnormal color or volume of urine. No preceding illness of any kind. He is tested for COVID weekly at college and was always negative including on 1-16-2021 He went to a freestanding ER where CT Abd without contrast showed normal appendix but mesenteric adenitis. White count was normal but creatinine was 2.8! He was given 2 L of fluid and recheck showed Cr 2.8 again. Urine dipstick 2+ proteinuria and microscopic hematuria. He works out 6 days/week but nothing unusual in the recent workout history. CPK was normal. Serum myoglobin slightly elevated at 94. He doesn?t take supplements, creatine, drugs, energy drinks, coffee or any medications. He was advised to hydrate copiously which he did. He was admitted for work up two days later. Repeat Cr was 3.1. ESR was 45 and CRP 2.26. RF, ANA, ANCA negative. Complement levels C3 and C4 normal. Uric acid and LDH normal. Amylase and Lipase normal. UA showed no protein or blood. IVF given overnight and kidney biopsy planned for the next day depending on that mornings labs. That morning Cr came down to 2.4. PTH was 127, Vit D 12.7, Calcium normal. SPEP normal and UPEP normal. Urine drug screen normal. Kidney biopsy cancelled and he was discharged. Cr two days later (2-10-2021) was 1.68

Within 5 minutes of receiving injection, while he was sitting in the 15 minute waiting area, patient had difficulty breathing. He started coughing. RPH injected him with dose of Epi Pen 0.3 mg & patient threw up after. We immediately called 911 and they came within minutes. Medics checked his vitals and all were within normal range.

At approximately 2 PM on Wednesday the 10th, I began to feel light headed, nauseous, and feverish. Color drained from my face and I also began to lose track of the conversation I was having. I couldn't keep up with the pace of the conversation and eventually realized I couldn't remember what we were talking about. Looking back on the event now, I could only remember what was happening for around. 2 minute span before I forgot it. After losing my ability to form new memories, I began to lose old ones. I would look through pictures and not recognize faces, listen to songs and not remember the artists, and lose entire gaps of my past. My parents described my memory loss as that of a concussion but, to be frank, I don't remember much of any of it. All of my past memories have returned, but I still have yet to regain my memory from 2 PM to 10 PM of that day.

patient "saw black"and nearly passed out was grey, sweaty HR 39, BP 96/60 after a few minutes his color came back after vomiting x2 observed in tent, BP 136/78, HR 69

Moderate to severe headache lasting roughly 4.5-5 hours Diarrhea and general gastrointestinal discomfort following. No nausea or vomiting.

Patient received 1st dose of Moderna at 11:06 am. Patient was able to walk to observation area and at 11:10 am, observation nurse noticed patient's face losing color and she was able to catch him and help him to the ground without him hitting his head. Charge nurse was notified and vitals taken at 11:13am B/P: 120/60, Temp.: 97.9, Resp.: 17, HR: 60, O2: 98%. Patient was able to sit up in chair on his own. After a few minutes , patient said he felt nauseous and vomited. Water was given to patient and he also drank some Gatorade. At 11:20 am patient went to sit outside with the charge nurse, he was able to walk steadily and without incident. After waiting a total of 30 minutes patient said he felt "much better" and was able to leave with his parent who was driving him home. Patient was able to stand and walk steadily without assistance.

Reports onset of throat swelling and difficulty swallowing which started app. 30 minutes after vaccine administered. Not reported to clinic until following day (4/8).

Patient received vaccine in left arm and during waiting/observation period a few minutes later (while sitting), he told the person near him he felt dizzy, and he fainted. He soon regained consciousness (within about a minute) and was observed/attended to. 911 was called immediately as a precaution, and paramedics monitored his vital signs slowly helped him into a chair. He drank some water and a little electrolyte drink. He did not want to seek hospital treatment, and paramedics left. He stated he was experiencing nausea while sitting in the chair, and said he started to feel like he was going to faint again. Again 911 was called and paramedics attended to him, but he did not faint again. After resting in a chair and drinking some more water and some electrolyte drink, as well as cold compresses on his forehead and the back of the neck, he was eventually escorted by two store employees on a motorized scooter cart to his parent's car and went with his parent home (he did not want to seek hospital treatment).

Nausea: Throughout evening Vomiting: A few times in a single 5 minute period at night; nausea stopped afterwards

dizziness/light headed, cold, clammy and sweating in face, fainted, then vomited. Occurred 4 hours after shot was given.

The patient reported feeling somewhat dizzy at the time of injection, this resolved within minutes. At around 45 minutes after administration, the patient reported what he described as mild pain in the right upper and lower quadrants of his abdomen. He was referred to a medical facility for further evaluation.

Patient called today, received the vaccine at a pharmacy, at 11 am. At 10pm now feeling like he cannot swallow, pale, chest pain, irritated throat. Was dispositioned by On call Provider Dr to seen medical care in the ED.

History of Present Illness: Patient is a 18 y.o. male with no pertinent past medical history who presents to the ED with complaints of chest pain. The patient notes that he started noticing chest pain this AM after he woke up that is associated with nausea. The pain is described to be in the center of his chest and is persistent. EMS was called who administered 2 SL NTG and 1 dose of ASA 325 mg. He notes that he received his 2nd Moderna vaccine on Thursday April 15th. He is currently a senior in High school. He had no adverse reactions to his 1st dose of the Moderna vaccine but after the 2nd dose he notes that he was having fever, malaise, body aches, and decreased appetite. He also tells me that he has been so fatigued that he has not really gotten out of bed in the last 48 hours. In regards to his oral intake he had (1) 16.9 oz bottle of water yesterday and nothing else but did have a glass of orange juice and some cereal this morning. During my evaluation with the patient he notes that he is still having chest pain that he reports to be retrosternal any 5/10 in severity. He denies any radiation into the left arm or jaw. He feels the pain is worse with movement and better when he is resting. He has had associated nausea and shortness of breath. He has been having low-grade fevers at home but denies any coughing, abdominal pain, diarrhea, dysuria, burning or itching with urination, congestion, rhinorrhea. He denies any tobacco use, drug use, alcohol use. In the emergency department initial workup is notable for potassium 3.6, magnesium 1.7, phosphorus less than 1, troponin 7206 with repeat at 10,212. CPK noted to be 380. Initial EKG was nonischemic. COVID swab was noted to be negative. After I evaluated the patient around 1520 an RRT was called due to hypotension and bradycardia. His blood pressure had dropped to 80/40 and heart rate was in the low 50s and he was symptomatic with diaphoresis and feeling as if he was going to pass out. 1L fluid was given running wide open. Repeat EKG did show some diffuse ST segment elevations concerning for pericarditis. Cardiology and ICU team were notified and evaluated the patient. Response is likely that the be vasovagal from acute pain. Patient stated prior to administartion of vaccine that he has had a history of passing out following vaccinations, mostly when he does not eat or drink beforehand. He stated he made sure to have something to eat and drink beforehand and was aware that it may happen following this vaccine as well. Prior to administering vaccine, I verified details with patient on previous episodes and discussed risks with patient, who understood and wished to proceed anyways and was okay staying additional time beyond 15 mins post vaccination to ensure no serious reaction. Immediately post-vaccination, I asked the patient how they were feeling prior to sending him to the designated waiting area. He stated repeatedly that he was fine and okay to walk to the chairs on his own. Moments after sitting down, a technician alerted me that he had collapsed off the chair and fell on the ground before anyone could get there to catch him. I immediately called 911 and attended to the patient who had already regained consciousness while I was on the phone with emergency services. He informed technician that when he sat down he felt a little queasy and the next thing he knew, he was on the floor. Fortunately there happened to be an EMT in the store that came over to help while waiting for additional paramedics. We provided the patient with an ice pack and a bottle of water, but instructed him not to drink anything until paramedics arrived. We checked the patients blood pressure and found the reading to be low and patient was tachycardic, expected following a syncopal episode. Myself and the EMT got patient to lie down on the floor and elevated his head. We noticed that the patient had suffered a small laceration to his head resulting from falling off the chair onto the ground. Store management also got involved to assess the situation and take statements. Upon arrival of paramedics, care was transferred over to them. At first, patient refused to be transported to the hospital but asked that we contact his mother. After reaching his mother, she arrived shortly after to the store, and after further rest and evaluation, patient was transported to a local hospital for a further workup .Patient was only unconscious for 20 seconds or less, and the event occurred within 5 mins of vaccine administration. Upon regaining consciousness, patient was alert and oriented, and again stated that he's fine and it wasn't the first time it has happened.

VR experienced nausea and vomiting a few minutes after vaccination. VR appeared diaphoretic and pale. Symptoms subsided after 5 min.

Patient felt dizzy about 10-15 minutes after the shot. He was alert and able to speak while sitting in the chair. Then he vomited a lot. Patient felt fine afterwards and sat for over 30 minutes. Had a history of fainting and vomiting after shots

18 YOWM PRESENTED TO PHARMACY FOR ADMINISTRATION OF 1ST DOSE OF MODERNA COVID VACCINATION. HE MENTIONED BEING "SCARED OF NEEDLES" BUT AWKNOWLEDGED THAT HE WAS ON BOARD AND WISHED TO RECEIVE THE VACCINATION. AFTER PT WAS IN OBSERVATION FOR HALF OF THE 15 MINUTES REQUIRED, HE STARTED LOOKING PALE AND CONFUSED. PT FAINTED, AMBULANCE WAS CALLED, PT AWOKE AFTER 5 SECONDS FROM WHEN HE FAINTED. NO SWELLING OF TONGUE, FACE OR THROAT WAS APPARENT. PT STARTED TO VOMIT. ADMINISTERED 25MG OF ORAL DIPHENDYRAMINE SYRUP. AMBULANCE ARRIVED, CHECKED VITALS WHERE ALL WITHIN NORMAL RANGE. PT WAS PICKED UP FROM FACILITY BY HIS MOTHER. CALLED PT TO CHECK ON A FEW HOURS LATER, HE STATED HE WAS DOING WELL.

Immediate diarrhea and nausea 15 minutes after first shot, then headache a few hours later. The diarrhea lasted for 5 days. The nausea and headache went away the day after the shot.

Patient received Moderna Covid-19 vaccine and fainted approximately 5 minutes later. Patient reported feeling dizzy, hot and nauseous. Patient's blood pressure 81/41mmHg, heart rate 58 bpm, oxygen level 96% (taken by paramedics) approximately 10 minutes post-vaccination. Paramedics took patient to emergency department to be monitored. Patient was given IV fluids and discharged from the hospital a few hours later. Patient reported that he has a needle phobia and had fainting incident post-vaccination years ago.

Patient received first dose of Moderna COVID-19 vaccine on 4/7/2021. After vaccine was given, patient complained of lightheadedness, numbness in hands, light sensitivity, and nausea. Doctors and an RN evaluated patient. BP 96/64, not tachycardic. Mother was driving vehicle. No meds given but patient sent with EMS to ED.

Between 5 and 10 minutes after the immunization, patient sat down with his father and starting feeling ill with a headache. Head was shaking a bit and the patient vomited. He was given some water while the paramedics were on their way. Patient was coherent and talking fine sitting in the chair. He was asked if he gets seizures ever and he both him and his dad said no. Dad said he just doesn't like needles. No issues giving the shot at all. He didn't look and said he hardly felt it. Paramedics arrived, patient still had a bit of headache. He vomited a small amount before he left with his father.

Pt received vaccine 4/20/21; developed profound HA, lethargy, diarrhea with symptoms persisting 7 days. CRP elevated, platelets decreased. lead to hospital admission. likely only 24 hours

PATIENT STARTED FEELING HOT, NAUSEA, TINGLING IN FINGERS AND FEET, HANDS WERE RIGID. HE WAS VERY UPSET AND CRYING. HE HAD PAIN IN HIS HANDS. EMT THOUGHT HE WAS HAVING A PANIC ATTACK

patient caregiver reported he experienced severe headache, nausea, body ache ,chills chest tightness and went to emergecy room in hospital and by the time patient was seen by dr he started feeling better so he was discharged from hospital after monitoring for some time

About 6 hours after the shot, I started getting pain in my arm. 2 hours later, I woke up and was severely shivering for two hours straight. In the morning, I woke up again with a fever of 104 and I dry heaved. My fever broke, but then a few hours later I got another low grade fever. I felt better about 24 hours after my shot.

After receiving first does of Moderna vaccine , patient became lightheaded and nauseous - symptoms resolved after patient placed briefly in Trendelenburg and drank 16 ounces of water , ate bag of chips. Patient was discharged form vaccination site with mother , symptom free

Patient experienced soreness at injection site, high fever (102 for 3 days), severe headache, muscle pain, nausea and vomiting, stomachache, extreme fatigue, palor. All these symptoms persisted for approximately 7 days. On the 8th day, patient developed strep throat as diagnosed by his physician and was given antibiotics.

Client was vaccinated with the Moderna vaccine at 1600, around 1605 while waiting in the observation area, the client vomited once and experienced some dizziness. EMS attended to client, and an ambulance was called. Vital signs were all WNL. After laying down for 15 minutes the client felt well and denied EMS transport to the hospital. Client left with family member in no apparent distress.

Patient got nauseous and clammy post vaccination. He was nervous and worked up prior to. We put his feet up and gave him some water. He felt better in 15 minutes and went back to work.

Pulsatile Tinnitus, verigo, dizziness, nausea, vomiting. When I searched the Internet, I found hundreds if not thousands of people reporting serious problems regarding their ears shortly after recieving a Covid-19 vacinne. Total or partial hearing loss, tennitis, vertigo, nausea,etc. This needs to be reported to the Public. I'm angry that I was not warned about this before getting the vacinne.

Major abdominal pain. Large amounts of extremely painful gas. Wailing in pain. Difficult to find comfort.

Dose 1: 4/29/2021 Dose 2: 5/27/2021 Symptoms onset began on 4/28/2021 and included fevers, cough, weakness, heartache, diarrhea, headache, dizziness. Symptoms temporarily improved on 5/29/2021. On 5/30/2021 at approximately 1:00 AM patient experienced constant, pleuritic chest pain/pressure and shortness of breath which prompted him to go to the emergency department.

Patient received his second dose Moderna and sat in our waiting area. He got up to tell us he was feeling nauseous and dizzy and he passed out. We lifted his head and legs and called EMS. EMS came and patient was ok. He passed out again when they checked his blood sugar but was seated and did not fall.

4/23/21 at around 1pm, felt pain in left shoulder blade and around left upper lung and ribs area. Felt like tissues around it was tearing and grinding. After an hour felt pain near heart area and experienced nausia and felt faint. Was taken to hospital around 3pm. Was screened and tested for Covid-19 - negative. XRay was taken - did not show any issues Was given strong anti-inflammatory via shot Was monitored for 2 hours. Sent home. Discomfort improved over the weekend and no longer experiencing any symptoms.

The occurrence of additional non-serious events is detailed below. No Medical History information was reported. On 01-Jun-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 01-Jun-2021, the patient experienced CONCUSSION (Concussion), TINNITUS (Buzzing in ears), EATING DISORDER (Did not eat), CHILLS (Shiver), HEADACHE (Headache), NAUSEA (Nausea) and VOMITING (Throw up). On 02-Jun-2021, the patient experienced ASTHENIA (Little weak) and FEELING ABNORMAL (Foggy head). On an unknown date, the patient experienced LOSS OF CONSCIOUSNESS (Woke up on floor/ Past out) (seriousness criterion medically significant), ANXIETY (Anxiety) and FALL (Fall). At the time of the report, LOSS OF CONSCIOUSNESS (Woke up on floor/ Past out), CONCUSSION (Concussion), TINNITUS (Buzzing in ears), EATING DISORDER (Did not eat), ASTHENIA (Little weak), FEELING ABNORMAL (Foggy head), ANXIETY (Anxiety), FALL (Fall), CHILLS (Shiver), HEADACHE (Headache), NAUSEA (Nausea) and VOMITING (Throw up) outcome was unknown. DIAGNOSTIC RESULTS (normal ranges are provided in parenthesis if available): On 01-Jun-2021, Blood glucose abnormal: abnormal (abnormal) abnormal. Patient presented to hospital on 6/20/21 with complaints of substernal chest pain, nausea and vomiting 6 days after his second dose of Moderna Vaccine. His symptoms had started on the day of his second vaccine dose ( 6/14/21). He had initially presented to another hospital on the day his symptoms began and was discharged home after observation overnight. He presented to healthcare facility on 6/20 because his symptoms of chest pain, nausea and vomiting had not resolved. On presentation, his HR was 39. EKG showed sinus bradycardia, Left axis deviation and T wave inversions in II, III and aVF. His COVID and Flu RT PCR was negative. HS Troponin was elevated to 168 and peaked at 283. There were no previous values to compare with from his prior admission. Cardiac MRI was done but did not show LGE. He was diagnosed with clinical myocarditis after Moderna vaccine administration.

Patient fainted approximately 15 minutes after administration. He felt nauseated immediately before, then fainted, falling to the floor and striking his head.

2 days after the vaccine, developed Right Lower Quadrant pain - varying in severity from 2 to 7 out of 10, intermittent. Suspected mesenteric adenitis, possibly secondary to vaccination. DDx includes appendicitis. Plan - observe at home, follow up if worsening symptoms or not resolving later this week.

Patient woke up with red spots/sores on tongue (likened to "burnt spots when you drink something too hot"), and they continue to persist. Patient has had trouble eating or drinking, as that increases the burning/tingling feeling.

Near syncope event after vaccine. Was sitting and turned pale and felt nauseated. Was able to lie down and was then feeling better. Was able to eat and drink and was discharged from the vaccination area in stable condition.

18-year-old male with high fever, headaches, malaise, body ache, and cough productive of green phlegm. He denies shortness of breath or chest pain and no blood in sputum. His appetite have been okay no vomiting or nausea but had 4 episodes of loose stool this morning. Patient had positive sick contact with Covid-19 infection. No other complaint today.

Pt grandmother reported him feeling dizzy, Never lost consciousness. Never had difficulty breathing. Vomited on the floor several mins following vaccine. Rescue was called per pt request. Cleared by EMTs.

PT'S FACE LOSE COLOAR . HE WAS VERY WHITE. HE ALSO HAD COLD SWEAT. HE FELT DIZZY AND HAD RINGING IN HIS EARS HE FELT LIKE HE WANTED TO THROW UP. HIS AUNT WANTED US TO CALL AMULANCE. THE PARAMEDIC CAME . ALL THE VITALS WERE NORMAL. HIS FACE COLOR STARTED COMING BACK. PARAMEDIC ALSO CHECKED IF THERE WERE RASHES ON HIS CHEST AND ARMS. NOTHING. PARA MEDIC ASKED PT WHETHER HE WANTED TO GO HOSPITAL, HE DECLINED. HE SAT 10 MORE MINUTES FOR OBSERVATION AND HE FELT WELL ENOUGH AND LEFT.

SYMPTOMS STARTED ABOUT 3 WEEKS AFTER RECEIVING SECOND MODERNA VACCINE. MONONUCLEOSIS WAS NOT DIAGNOSED PROPERLY UNTIL TWO WEEKS AFTER SYMPTOM ONSET. SYMPTOMS INCLUDED: CHRONIC FATIGUE LISTLESSNESS NO APPETITE FEVER

Patient was given Moderna vaccine at approximately 16:30 hrs. Patient began to feel nauseous, but did not vomit.

Symptoms began 6/11/21, neck pain, intermittent stomach aches, chest pain described as "pressure". Treatment- colchicine 0.6mg BID x 3mo

Patient felt nauseous. He got up to go to restroom without telling staff. He apparently lost passed out on his feet. He fell but did not remember falling. He immediately regained consciousness. After feeling better, he sat in chair while he waited for family member to arrive. Upon leaving, he seemed to be better.

Patient reported Tiredness, headache, and nausea on day of vaccine (7-25-21). On 7-29-21 pt woke up with body rash.

I got the injection at 3:35pm on 1/20 in my left arm. Within the first 30 minutes, my shoulder around the injection site was incredibly sore, my left elbow had a lot of joint pain, and my left thumb and index fingers went numb within the first 30 minutes, I woke up at 8:00am on 1/21 and noticed that I had a light headache, felt extremely groggy, and was experiencing an uncomfortable amount of muscular and joint pain. I went to work at 9:00 and on the drive there, I began to feel dizzy and I felt as if when I turned my head, my vision would lag behind and take a second to process my surroundings. By 10:00, my muscular and joint pains were significantly amplified, my headaches were increasing in frequency and amplitude. I also began to have the chills, followed by periods of radiating heat from my body. Coworkers told me I looked very flushed, however, I never developed a fever. By 11:00, the muscular and joint pains, coupled with the headaches forced me to sit down and I almost broke down into tears. There were three sperate times where I began to get very lightheaded and thought I was going to pass out ( I did not). I went home at noon and noticed that my muscles felt like they were on fire. I took Tylenol and Advil whenever I could, which helped to reduce the symptoms. we had dinner at 7:00, however, I had no appetite, without eating since noon. On 1/22, I woke up at 8:30 with a pounding headache, muscular and joint pains still there, however reduced, and the same groggy feeling. This morning, I also noticed that I had a very sharp stomach pain in my abdomen. I also woke up with a severe nosebleed, which is extremely uncommon for me.

Immediately after the vaccine was administered I experienced the usual side effects of mild headache, fatigue and localized redness and swelling Around 11pm the night the vaccine was administered I developed a migraine that resulted in blurred vision. and when I checked my temperature it was approximately 104.9 degrees Fahrenheit.By the next morning, approximately 7am, I was weak to the point I could not get out of bed myself, I was nauseous that resulted in a single Emesis episode approximately 8am, I had mild swelling of my face since 7am, between 7:30am and 12pm, I had multiple times where I would temporarily lose my vision. Throughout 1/25 I continued to have a fever ranging from 102 degrees Fahrenheit to 103 degrees Fahrenheit. At approximately 8am and 1pm I took two acetaminophen pills each time which alleviated the migraine and to an extent helped with the visual deficit episodes. Throughout 1/24 and 1/25 I suffered with extreme chills resulting in shivering that lasted for several minutes despite a room temperature of approximately 90 degrees Fahrenheit and covered in two blankets. PCP was consulted via telephone and she advised to go to the hospital via EMS given the high grade fever, abnormal vitals and severity of symptoms

I called my insurance provider's RN hotline at the onset of dizziness, which was then followed by a bout of ear ringing and nausea. I was advised to seek immediate medical attention, so I had an RA at my university dorm walk with me across the street to M Health Fairview hospital. I was seen by an RN in their emergency room, but was ultimately never seen by a Doctor as I did not present a clear risk of anaphylaxis at the time. After waiting in the ER for four hours for medical care, I ultimately left because most of my symptoms subsided. My most lasting symptoms cleared up late yesterday (1/25) and they were extreme soreness/tenderness at injection site and fatigue.

19 yo medically complex M with prior COVID infection in Dec 2020 who received first dose of the Moderna COVID vaccine on 3/4 as well as Epo shot (for chronic anemia secondary to CKD). Recently admitted for MRSA sacral abscess and had recently been discharged to complete treatment on oral Bactrim. On 3/6, developed worsening diarrhea, decreased urine output, facial and tongue swelling, and altered mental status. EMS was called and patient was brought to the ED. Upon presentation, noted to have hypotension and was fluid resuscitated. Initial blood gas with hypercarbia, and he was intubated for airway protection. CXR was non-focal. He was started on empiric vancomycin and ceftriaxone, although these were discontinued with negative blood and urine cultures. Repeat respiratory film array was negative. Patient intubated 3/6-3/10.

Starting that night, and lasting for about two days after, i would get a 10-15 minute flash of illness every few hours. Symptoms consisted of nausea, sweating, lightheadedness, and tiredness.

After patient received COVID vaccination, he exhibited seizure like activity. Staff moved patient to floor. 911 was dispatched. Patient complained of nausea. Vomited x2. Skin pale and diaphoretic.

Immediately after receiving dose patient began to feel dizzy, light headed and nauseas. He looked pale and sweaty. Upon further examination his BP was low at 70/53. He stated he had not eaten that day. We gave him water, juice and pretzels and he began to feel better in 10 minutes. His BP was retaken 15 minutes later and it re-elevated to 122/78. He phoned a family member for a ride home and the supervisor was notified. Pt was instructed to go on V-Safe to report and was counseled.

Severe abdominal cramping, vomiting 2 days, lower back pain, elevated blood sugar (159 after 18 hours post-prandial and having vomited all night with no history of diabetes), diarrhea, Chills

vasovagal response. - sudden flushing - faint feeling x30 seconds - passed out while seated (x3 seconds) - resolved unconsciousness after laying prone - nausea and dry heaving once conscious x1 minute return to normal after 5 minutes (and given 500mg tylenol & 25mg benedryl)

Patient started feeling nauseous and was feeling very faint. He did state that he ate before coming to get vaccinated.

The patient experienced 5/10 bilateral frontoparietal headache and nausea 4 days after receiving the vaccine. On the 7th day, CBC revealed low platelet count. The patient was subsequently admitted, vomiting and fever occurred during stay, platelet count monitored, given IV antibiotics. Symptoms resolved, platelet gradually went up, and the patient was discharged after 5 days.

Myalgias, chills, subjective fever and diffuse nonfocal abdominal pain without acute abdomen, N/V or D.

Approximately 2.5 hours post vaccination, patient felt extremely dizzy and nauseous. Patient was transported to hospital by ambulance for further evaluation.

Patient had parked in the COVID vaccine waiting area, and was observed to fall forward onto his steering wheel within a few minutes of parking by 2 other participants waiting in the car next to him. One of them immediately notified one of the parking lot volunteers who came and got me within 1 minute of this observation. 2 students and I came quickly to the open drivers side door and began talking to assess the situation. Mr. Bannister appeared pale, mildly diaphoretic (sweating) and slightly confused as to the passage of time, but was able to state and spell his name and talk coherently about his circumstances. He stated that he was not allergic to anything that he knew about, and that he had no prior history of similar reactions to vaccines or medicines although he had not taken any shots in several years. He noted that he had not eaten anything since breakfast (now mid-afternoon). He described himself as initially feeling weak, faint, and nauseated with his vaccinated arm feeling heavy and numb. He also reported that he had a sensation of blurriness before passing out, but denied any spinning sensation, visual loss or ringing in the ears. There were no obviously visible abrasions or bleeding on his face or forehead. The nursing students brought a chair, and we elevated his legs and gave him some water to drink during the time we were monitoring his vital signs. He refused a cupcake because of the nausea, but as time progressed he gained facial color and was more comfortable in conversing. His hands had equal muscle grip and strength, visible fingernail beds were pink and his capillary refill was less than 3 seconds. Towards the end of our monitoring, he did accept some nuts and M & Ms to eat. A University Safety Medical Report was completed by the appropriate safety personnel. He was fully AAO x 3 at the end of our monitoring, and stated that if he felt better he would drive to his apartment (less than 5 minutes away) but would wait for his girlfriend to get out of class if he did not feel fully capable of driving. We encouraged him to eat something light when he got home, to use cool water/warm water soaks at his shot site, exercise his arm, and to take an NSAID for pain relief the next day. We recommended that he eat something light before his second shot (which had already been scheduled), that he bring someone with him the next time, and as a precaution to wait 30 minutes following the second shot. He left at 3: 15 pm driving his car without any apparent difficulties. An online report was completed for the incident 04/20/2021.

9:59 AM - Vaccine 2:39 PM - Stinging pain in left arm which slowly radiates out from the vaccine site over the next several hours 7:25 PM - Pain reaches base of neck 7:29 PM - Nausea 7:30 PM - Lightheadedness 8:09 PM - Pain reaches tailbone 8:13 PM - Entire back and both shoulders begin throbbing 11:25 PM - nasal congestion 12:15 AM - body temperature drops to 97.9 12:20 AM - shortness of breath, blood oxygen drops to 95% 12:32 AM - blood oxygen drops to 92%. 12:34 AM - blood oxygen rebounds to 95% 12:35 AM - blood oxygen stabilizes at 96%. I am usually at 97-98 while resting. 1:31 AM - one of the worst sore throats I've had in my life begins 2:05 AM - low grade fever 99.1, chills, severe shivering even when under three blankets with heater on full blast, uncontrollable muscle spasms in face, arms, fingers, and legs 7:32 AM - woke up with severe muscle and joint pain. Too bad to sleep through. Can't even walk from my bed to my bedroom door without collapsing. High grade fever 101.7.

patient was sitting in a chair in the waiting area the 15 minutes when he fainted from the chair after 12 minutes. was nauseous and light headed still after 5 minutes. call 911 and paramedics checked him out. patient refused to go to hospital. mom took him home

Pt was waiting in 15 minute waiting area. Was doing fine then went unconscious-fainted, went to the floor slowly from the chair. Woke in about 15 seconds. Negative for head injury. Slightly nauseated, -HA, + shaking. Recovered, then -HA, -Shaking. HR was lower than his normal HR. Offered food/drink and patient was able to do this. Patient reported feeling better. Offered to call for a ride home, but patient denied, reporting that he felt "100% better".

Pallor, diaphoresis; cool and clammy; dizziness syncope; bradycardia; nausea and clear color emesis; Vital signs :BP- 102/60; 118/78; 116/76 . P-60; 70;76. Resp-16-20 .O2 sats 100%. treatment provide supine position elevation of lower extremities. Cool pack to head and back of neck . Client asymptomatic upon release. Skilled instructions provide to refrain for use on influential substances , as well not to be under the influence with 2nd dose appointment. Verbal understanding noted.

pt was very nervous before the vaccine, he was okay right after he received shot and commented it was not bad. After vaccine he was sitting in post vaccine area he got up and said feels very light head. I came out of the pharmacy to direct him back to chair while holding him , he started fainting we sat him down. Eye was rolling and unconscious his mom kept calling out to him . His color was pale. We called 911. he was conscious mom gave him gatorade he vomited right away but said he is feeling better. I kept rubbing his back. He said he was nervous all day and had not eaten .

c/o of nausea & lightheadedness 10 minutes fafter receiving vaccine. Slowly fell to the floor & fainted. Did NOT hit head. LOC for about 10-20 seconds. Upon arousal, alert and oriented x3. BP 84/59m HR56, pale and diaphoretic. 911 called. Repeat BP 103/65, HR 72. Ambulance arrived. Transported to ED.

began with nausea, then chills, sweating, fever, headache, muscle and body aches and pain, loss of appetite -began around 8 pm and lasted until Wednesday, May 12, when awakening that morning.

Immediate light headedness 1 minute post administration progressing to chest pain and tightness along with abdominal pain/cramping approximately 2 minutes later. EMT's contacted. Checked patient's blood pressure which had dropped to 96/50 mmHg. Provided water to patient 2 minutes later to re stabilize cardiovascular stability. Chest pain and lightheadedness ceased within 1 minute after providing 16 ounces of water. Checked blood pressure again which returned to 110/69 mmHg. EMT's arrived approximately 20 minutes after administration and patient was assessed. Blood pressure noted at 126/70 mmHg. Patient was allowed to go home with slight, residual abdominal pain.

Systemic: Dizziness / Lightheadness-Severe, Systemic: Flushed / Sweating-Medium, Systemic: Nausea-Medium, Additional Details: PATIENT LIGHTHEADED AND PALE AFTER ABOUT 10-15 MINS AFTER VACCINE GIVEN. PATIENT WAS RESPONSIVE. PATIENT WAS GIVEN ORANGE JUICE AND AN ICE PACK UNTIL EMT ARRIVED. EMT ARRIVED AND PT BLOOD PRESSURE WAS LOW. PT TAKEN TO NEAREST ER FOR FLUIDS.

After having the shot, hedeveloped left arm pain and did develop some lymphadenopathy in his left axillary region. He was dealing with nausea and dizziness, and the following day, he went to work, although he was feeling a bit ill. While at work, he was found to have a fever, so he was sent home. He has been having hot and cold flashes since then. He has had a poor appetite. He has had a dry mouth and trouble sleeping. One day prior to admission, he then developed a chest pain that became constant and persisted, so he came into the emergency department on 05/19/2021 for further care. He has developed myocarditis after receiving the first of his Moderna Coronavirus Disease 19 vaccines.

He started with fever and chills on the evening of the day he received the vaccine, Also severe pain in lower sternum, emesis \over the next 2-3 days developed abd pain of lower abd, cough, resp distress, waves of fatigue and hyperventilation, on 5/10 noted full body rash which was itchy, last fever was on 5/13/21, on 5/12 had blood shot eyes, 5/14 - peely skin on palms and face and felt thightness in face

5 minutes after getting vaccinated patient stated having issues seeing "seeing black" I offered to call 911 but he did not want me to. I got him candy and water. 5 minutes later he vomited - he said he felt better but I asked him to stay for continued observation. I called him this morning and he said he just had a sore arm

Patient became delirious, angry, and confused almost immediately after the second vaccine. He began throwing up at the sight or smell of food. Symptoms intensified throughout the day and he was taken to urgent care around 8:30-9pm on that same evening. EKG showed ?Pericarditis? and a CK level of 1,459. he was in ER overnight and admitted to the hospital on Friday, May 14th.

Patient passed out within 45 seconds of vaccine, convulsed for 5 secs while unconscious. woke up with in a minute, vomited after 3 minutes. He was able to say his name and address. BP 116/69 Pulse 68. patient said that this has happened before after an unknown booster at age 17. Only ate french fries earlier today. Was able to stand after 5 minutes. Put cool paper towel on neck, ate fruit bar and drank water before leaving

Developed acute onset chest pain and shortness of breath on 5/11 with nausea/vomiting x1. He didn't have any prior systemic symptoms, URI symptoms, rashes, and was in his usual state of health. Other ROS was negaitve other than longstanding right shoulder pain without weakness.

Patient received Moderna Covid-19 vaccination at 10:50 AM on June 2, 2021. Within 10 minutes patient describes being shaky. Patient was given orange juice. After 2-3 minutes patient described chest tightness which progressed into left arm pain during our conversation and was not improving. 911 was called. As paramedics arrived, approximately 11:30-11:40 AM, patient began to complain of trouble swallowing but no trouble breathing. Paramedic checked patients heart and patient started to develop hives. The paramedic gave the patient diphenhydramine and then was taken to the hospital for treatment.

Got vaccinated on May 22 on a saturday at store/pharmacy with Moderna. This was the second dose of the vaccine. I use a smart watch to monitor my health 24/7. I noticed an unusual elevation in my heart pulse after the first vaccine but that went away in a few days and I was back to normal. After the second vaccine I had the usual expected symptoms of headaches etc which went away the next day. After receiving the second dose on saturday I experienced unusual chest pains on monday which lasted for a few while and I was fine the rest of the day. It was tuesday when things got worse. I had a very sharp chest pain which I had never experienced and it continued till I took Alegra. I thought it was heartburn but when I went to the hospital they did my EKG which showed some abnormalities. I was then diagnosed with Pericarditis. After staying in the hospital for 4 days. I was discharged with anti inflammatory meds. I hope to have a full recovery in a few weeks.

Systemic: Fainting / Unresponsive-Mild, Additional Details: patient fainted immediately after receiving vaccine, seemed slightly disoriented initially upon waking up, felt nauseous, but symptoms rapidly improved after drinking water and eating crackers and remaining seated. Patient called mom to pick him up so that he was not at risk driving home alone.

Within 1 minute of receiving vaccination, patient became lightheaded and passed out. Patient regained consciousness within 1 minute of passing out. Upon regaining consciousness patient reported a headache, dizziness, shakiness, and nausea. The patient appeared pale. The patient was given water and peanut butter crackers. After 30 minutes patient returned to normal and was able to leave the pharmacy on his own.

Pt is a 19-year-old male with past medical history of hyperlipidemia, obesity who presents this hospital on 7/19/21 for chest pain and shortness of breath following administration of COVID-19 vaccination Moderna X 4 days. Patient noted to persistent substernal chest pain radiating to left upper extremity present upon exam at its worst 3/10 relieved with Advil and aspirin for which patient was taking daily following a COVID-19 vaccination Moderna first dose 7/15/2021. Patient stated pain persisted despite taking over-the-counter analgesics and had difficulty breathing today thus transported himself to the emergency department for further evaluation. Patient d admitted to subjective chills without recorded temperature, myalgias without rash, difficulty swallowing, palpitations, orthopnea, PND, increased weight gain, abdominal pain, nausea vomiting or diarrhea. Patient prior allergic reactions. Patient denied history of MI, TIA, CVA, thromboembolic events. Significant family history of mother MI and Graves' disease.

A few minutes after patient received the Moderna vaccine in his left shoulder, he reported that his left arm was numb down through his 'pinky' finger (although he was able to move it). He subsequently reported that he felt tired, he developed a headache, he had some stomach cramps and began to get nauseous. He did not ever vomit during this period. His blood pressure was monitored and found to be elevated initially at 173/121 (with a pulse of 69). His blood pressure was monitored over the course of just over an hour. It varied over this period but was elevated. The pharmacist in charge determined that patient was not able to go back to work. Patent stated that his girlfriend with whom he lives was off work today and could stay with him. He was advised to call 911 if he started feeling worse. Patient was driven to his home by a co-worker. Through the course of the day, contact was made with Patient and his girlfriend (per his authorization). He ate and then slept for some time. Upon awakening, he vomited for awhile - but "not alot". The numbness in his arm started subsiding. The headache maintained. They had no way to check his blood pressure. At 3:36pm he indicated he was "doing somewhat better" but the nausea was not any better.

Employee recieved the Maderna vaccination at 4pm 1/11/21. Employee was monitored for 30 minutes after vaccination and showed no side effects. He states that at 5pm he had a scratchy throat, at 8pm he had hives and his throat, chest and mouth started to itch. At that time he took Benadry and layed down. He woke up at 10pm with a swollen mouth and throat and was wheezy. He stated he had a hard time breathing and swollowing. At that time he took his EPI PEN. He did not seek medical treatment. Employee states that on 1/12/21 at 9:30am all symptoms had resolved.

lightheadedness and malaise developing day of immunization, wwith nausea at bedtime. awakened in the morning, had 2 episodes of vomiting, and one of diarrhea

Patient received vaccine on 1/13/21; started having headaches on 1/13/21 in the evening. On 1/14/21 developed chills, body aches and nausea. On 1/15/21 had chills, body aches, nausea and fevers as high as 102.0F - also developed lump in left neck on 1/15. Starting on 1/16/21 he noted pain radiating from left deltoid to left hand digits - specifically the 4th digit, palmar surface. Symptoms began improving on 1/17/21 and on that day he mainly felt just soreness in his left arm. On 1/18/21 he noticed the lump in the left neck was still present and he had tightness in the left triceps region.

Chills and bodyache following the #2 of moderna. Pt is tachycardic. Otherwise unlabored breathing. Steady gaits. 20 yo M w/ no known chronic medical conditions, p/w frontal HA, dry mouth, myalgias, fatigue, and nausea since last night. Received the COVID vaccine yesterday afternoon. No photophobia, vomiting, diarrhea, abd pain, cough/congestion, CP, SOB.

Started with a fever that slowly worsened to a high temperature. This resulted in chills, aches, pains, fatigue. nausea, congestion, sore throat, headaches, and weakness. There was also swelling at the injection site and swelling of my eyelids.

Fever with tMax 103, Chills, Sore throat, Cough, headache, vomiting, Malaise. Lasted about 2 days and gradually resolved. Lingering headache and dizziness. Dizziness worsened 1 week after vaccination, progressing to Vertigo with Vomiting twice, required ER visit and treatment.

The above patient received a COVID-19 vaccine on 03MAR21 @ 1440. He came to the EH&S office on the morning of 04MAR21 complaining of left arm pain, fatigue, headache, muscle pain, joint pain, chills, and nausea/vomiting. He was evaluated by a healthcare provider who noted mild swelling and a small knot was felt in his left arm. He is to follow-up with our onsite nurse or his medical provider if his condition worsens.

Client received 1st dose vaccine and remained in post vaccine monitoring, at the 15 minute point (end of monitoring) before leaving client reported feeling very dizzy with blurred vision. On assessment client was sitting up in chair, alert and oriented, V/S HR 72, RR 20, BP 124/72 O2 sat 98%. Lungs CTA, no reported difficulty with breathing. Client reported additional symptoms of itching, no rash noted and no change in skin color or appearance; nausea was also reported at this time. Information obtained from client on medical history, vaccine history, allergies and current medications. During this time period the itching and nausea was relieved, but the dizziness and blurred vision remained; V/S HR 74, RR 20, BP 128/78 O2 sat 98% . Client reported he started on a new medication Hydroxizine approximately 2 weeks ago and did experience dizziness in the first 2-3 days of starting the medication, but has not had that occur since that time. Client was unable to stand initially for BP check while standing, at 0945 additional medical assistance available to assist: sitting BP 130/76 HR70, standing BP 124/70 HR 70. Client reported no change but states dizziness and blurred vision continued and now he is seeing with blurred vision in what he reported at "cool and warm colors - like a filter on a phone". EMS was called at 0959, arrived at 1011 to evaluate and transport client to the hospital for evaluation. Client departed with EMS on a stretcher at 1014am.

Patient presented to the medical with a complaint of nausea/vomiting, diffuse muscle aches, fatigue, and headache which started the night after he received his vaccine. His vital signs were normal and his exam was unremarkable. He was provided symptomatic treatment with acetaminophen and ibuprofen and asked to follow-up if his symptoms fail to improve after two days.

Patient had vaccination and placed in observation area. Patient had syncopal episode post vaccination. Was seen fall down and hit head and had seizure activity that lasted less than 10 seconds. Patient had brief postictal phase without loss of bladder or bowel control. After a few seconds patient was able to relay his name and was alert and oriented to time and place. Evaluated patient for bodily harm, after finding none moved patient to medical area. Took vital signs and called for transportation to hospital. Medical transport came and after evaluation asked to take patient to hospital patient refused transportation. Patient refused going to hospital and left AMA. Of note, Patient has a history of syncope post injection that was failed to inform us. He mentioned he was afraid to needles like most patients report. Patient said it has happened before after flu shot along with syncope and seizures.

While in observation, patient reported feeling nauseous and was mildly diaphoretic. Vilas taken, 95/58 bp at the time. Patient then began to complain of chest pain. Vital signs taken again, as vital signs were being taken, pt reported relief from both nausea and chest pain. pt observation extended to 30 minutes. @ more vitals completed, all within normal limits. Pt refused further evaluation by md. Patient left facility with no complaints and a steady gait.

Fever, chills, stomach pain, nausea, vomiting, tiredness, muscle aches/pain, migraine. Lasted about two days

Patient reported stating feeling nauseous, lightheadedness, paleness, dizziness, and had trouble seeing to observation nurse, at 1116 AM. Vaccination nurse went to notify charge nurse on patient status. Vital signs included T 96.1, o2 99, RR 16, B/P 106/76, and HR 52 at 1116 AM. Supervisor on site notified the doctor via telephone at 1144 AM. Reassessed at 1126 AM second set of vital signs includes T 96.1, o2 98, RR 18, B/P 94/74, and HR 77.Currently patient does not display any s/s of distress or discomfort third set of vital signs includes T 97.0, o2 98, RR 18, B/P 116/86, and HR 78. As per MD patient is stable and can go home, clear to leave observation area. Patient instructed to contact HCP and/or go to ER if displays s/s.

Client complained of lightheadedness and nausea. He then complained of tingling all over his body. Client reported that he did not eat breakfast or lunch.

My 20-year-old son received his first Moderna shot on March 16, 2021, at his college health clinic with no obvious side effects. He received the second Moderna shot at the same location on April 13, 2021. On the morning of April 14, 2021, he began running a high fever with repetitive vomiting and stomach cramping. He did not immediately seek medical attention because he had been told of side effects of the vaccine. When it was no longer tolerable, he reported to the local ER where it was later discovered he had a 4 mm kidney stone, which he passed two days later. We were told that the kidney stone would likely have begun forming about 31 days earlier, which would approximately coincide with his first COVID injection. There is no family history on either side of any kidney stones in the family. While my son's lifestyle may introduce some risk factors for kidney stones, I felt it was reasonable to report it as potentially relating to the vaccine.

General tenderness in injection site day of and day after, nausea and vomiting two days following. Woke up with no pain or discomfort, but as I stood and walked I was in pain from the injection site to my stomach, then I proceeded to vomit and felt fine.

At 5:00pm. Blood pressure was elevated to 137/91, No temperature noted even though he felt hot and clammy. Gave him water, and put legs on a pillow and he lay down At 6:30 he began to feel sweaty, clammy and included nausea. These episodes continues until about 8:30pm when he started to feel a bit better After a night of sleep that was restless and about 6hr long woke to feeling better but tired.

symptoms fever, chills, and fatigue first occurred 4/9 shortly after receiving shot, and lasted until 4/13 on 4/10 vomiting and temporary blindness after exercise occurred. the blindness episode did not last longer than 2-3min, but was described as complete vision loss and occurred 1-2min after intercourse. on 4/14 an incident of ?blacking out? occurred during prolonged intense exercise

Admission HPI: Right-sided chest pain began 2 days prior to admission (3/19) after getting out of car, without identifiable trigger. Pain was sharp, pleuritic, and worst with movement; didn't feel it when still. Pain not responsive to ibuprofen or heating packs. Reported 30-60 seconds of self-resolving heart racing during ocular migraine which has been his baseline for years. Not associated with SOB (at rest or with exercise), palpitations, presyncope, or syncope. No viral URI symptoms, N/V, or rash. He did receive his 2nd Moderna COVID vaccine about 1 week PTA, followed by 36hrs of fevers, chills, and body aches. Given persistence of pain, Patient asked his parents to present to urgent care. In UC, he was noted to tachycardic with CXR with bibasilar atelectasis and b/l effusions, per report. Given continued tachycardia, decision was made to transfer to ED to r/o PE. In ED, tachycardic to 104-116 and tachypneic 22-26. Had one episode of tachycardia to 182 iso ocular migraine. Endorsed chest pain but points to R middle abdomen when asked where worst. Exam was unremarkable. He received 1 dose of morphine which reduced pain from 10/10 to 0/10. Labs notable for leukocytosis (14.61), thrombocytopenia (422), elevated d-dimer (1.56), and elevated CRP (5.6). Otherwise normal chem and coags. COVID/RVP negative. CXR unremarkable. CTPA showed scattered PEs in b/l lower lobes with small pulmonary infarct in RML. No evidence of right heart strain. Heme was consulted who recommended starting him on lovenox and sending lupus anti-coagulant. Cardiology was consulted who recommended EKG, BNP, and troponin, all of which were normal and reassuring against right heart strain. ECHO was therefore deferred to the AM. The decision was made to admit. Upon arrival to the floor, Patient was tachycardic 114 and tachypneic to 22, otherwise stable vitals. He denied any pain since morphine. Denied any unexpected weight loss; reports that car ride was short. Mom reports that Patient has been less active d/t cold weather and baseball being on hold. Denies family or personal history of clots. Last seizure ~13 years ago. Hospital Course: (3/21 - 3/22) Patient was admitted to the hematology service. He was therapeutically anticoagulated with LMWH injections and his anti-Xa level prior to discharge was 0.92. (goal 0.5-1.0) Lower extremity ultrasound with Doppler showed patent veins and no evidence of DVT and upper extremity ultrasound showed no evidence of DVT, though there was potential evidence of collateralization seen in mid and distal third of right subclavian vein and left internal jugular vein. Since this was an unprovoked PE, a full thrombophilia workup was sent and will be followed up by the outpatient hematology clinic. Cardiology consulted regarding possibility of right heart strain and the tachycardia during migraine episodes. Echocardiogram performed on 3/22 was technically limited but showed no gross abnormalities. The cardiology team requested a repeat echo scheduled outpatient for 1-2 months after discharge.

Patient had bowel movement after waking up and going to bathroom. He fainted, calling for help to his parents on his way down to the floor. Parents found him on floor cognitively responsives but not moving body (He does not remember anything after coming to kneel on floor). Parents found him clammy. Mother called 911 and was instructed to keep him lying on floor. He complained of being cold but was sweaty. Blanket placed over him on floor and kept there until EMS arrived. Patient c/o nausea. They got him into rolling chair and was going to leave when he had another episode of dizziness, clammy and nausea. EMS transported him to hospital. Blood work was done with no abnomalities noted. Upon discharge he complained of headache. Nausea dissipated upon eating and some ginger ale. Patient had previous had "covid arm" with first injection but no other complaints.

Arm soreness, tinnitus is much louder, dizziness, nausea, insomnia, fatigue, loss of appetite, diarrhea, On 4/27/21 I went to urgent care, they ran blood tests which showed possible signs of viral infection, was only prescribed zofran for the nausea and vomiting. Ears showed no outward sign of irritation or infection

I received the vaccine with no issues, within a few hours i experienced a minor headache. I went to class and woke up the next morning cold. I couldn?t get out of bed, I was weak, Nauseous, head was pounding. I was brought 800mg of ibroprofen and a thermometer. I temperature was a 103.7. I monitored my temperature all day long. My fever broke around noon, still sick. Around 3:00pm it returned at a 100.6. I went to bed for the day with the fever after taking more ibroprofen later that day and woke the next morning with a headache but other than that without symptoms. On today (May 4th) I still have a headache, but without symptom.

The occurrence of additional non-serious events is detailed below. No Medical History information was reported. On 25-Apr-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 25-Apr-2021, the patient experienced LOSS OF CONSCIOUSNESS (Passed out) (seriousness criterion medically significant), WRIST FRACTURE (broke his wrist), DIZZINESS (Extremely dizzy) and NAUSEA (Nauseated). At the time of the report, LOSS OF CONSCIOUSNESS (Passed out), WRIST FRACTURE (broke his wrist), DIZZINESS (Extremely dizzy) and NAUSEA (Nauseated) outcome was unknown. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) was unknown. No relevant concomitant medications were reported. No treatment information was provided. Company Comment: Based on the current available information and temporal association between the use of the product and the start date of the events ( loss of consciousness, dizziness and nausea), a causal relationship cannot be excluded. Approx 5 min after receiving vaccine, pt felt faint and nauseous, was pale and sweating. Helped pt to lie flat on floor with legs elavated, applied cool compress to forehead. Pt stated feeling better immediately. After a period of time, helped pt back to chair to wait with companion.

12 minutes after injection - local itching/redness. 30 min. after vaccination anaphylaxis with multiple organ involvement - skin injection site and uvula swelling and eyes itchy. Patient had angioedema of throat with swallowing difficulty. Had mild asthma/wheeze. Also had tachycardia and light headed when trying to sit up. Patient given Epi 0.3 ml IM, Benadryl 50 mg IM, and Pepcid 20 mg PO. Transported to hospital by EMS for 4-6 hour observation. Patient responded well to epi.

Patient was nauseous and went to the restroom. I had my tech, follow him in there to ensure he was okay. The patient was trying to vomit but nothing came out, all he had was a gagging reflex. The patient asked for water so we gave it to him. He stated he felt better when his parents came to meet him up. He stayed past 15 minutes for observation to ensure he was okay.

patient developed chills, headaches, fatigue and unable to do normal school work tasks for 2 days due to weakness and nausea.

patient lost consciousness about 30 seconds after receiving the vaccine. Initially I treated it as a syncopal episode and was just trying to wake him up but he began to shake (seizure-like) after about 15 seconds. I could not rule out anaphylaxis so I administered a dose of epinephrine via epipen. Patient regained consciousness approximately 5 to 10 seconds after epipen. EMS arrived approximately 10 minutes later and took patient to the hospital for further observation. Patient vomited on ambulance ride but did not lose consciousness again. Patient reportedly fine after being administered fluids but reports no memory of ambulance ride or even being vaccinated.

I ran a fever, was in cold sweats all night long, vomited, and had an extremely high heart rate. I have honestly never been so sick in my life but it did settle out a bit. Now I just feel exhausted and have a headache. I've been feeling better and better but I felt as though it was worth reporting.

At 1:50 PM the patient stated that he felt lightheaded, sweating, high heart rate, nausea; Vital signs = BP 118/68 BS73, HR-90, Res-20; Treatment cold water, laid on the ground, ate a banana, called EMS. I have ulcerative colitis. I had my first loose/bloody stool the day after taking the vaccine after being in remission for around 1 year on Entyvio. Following the day after the vaccine, I continued to have worsening diarrhea, blood in stool, and abdominal cramping. This being a UC flare was supported with negative C. diff, negative ova and parasite, and elevated fecal calprotectin of 791 ug/g. This forced me to limit my diet, and I consequently lost around 30 lbs. Because these symptoms were occurring despite a normal vedolizumab level and no antibodies, I was forced to start a course of prednisone, which I had not had to do previously for a year.

Asked if he needed to be evaluated and he said yes. EMS came and evaluated and he was then able to walk out of the store and refused to be brought to the hospital. Patient was completely fine and walked out.

I had chest pain for about five days after receiving my second dose of the vaccine. It felt like intense and sporadic heartburn. I went to the ER on the third day of experiencing the pain, during which they performed an EKG and bloodwork in order to determine whether or not it was a heart issue. There were no significant results and the pain subsided two days later.

Patient became dizzy, nauseous, and had difficulty seeing approximately 15 minutes after his vaccine. Checked patients blood pressure which was low 70s/50s mmhg initially, patient remained conscious the entire time. He had no trouble breathing or swallowing. Patient was given a cold water and stated this helped. after initial BP reading EMS was called. Patients BP gradually rose and was 110/74 mmhg when EMS arrived. Glucose was also tested and was 101. Patient was feeling much better and declined transport to the hospital.

I felt extreme chills at night, and could not sleep even with a ton of clothing and layers on, and even after taking sleep medication. Also, nausea, and fatigue (but I think fatigue is due to lack of sleep)

Pt received his first dose of the Moderna Covid-19 vaccine. Immediately following the injection the pt began convulsing. Convulsions lasted for less than 30 seconds. Upon completion of convulsions pt was awake and alert. He stated he was thirsty. Before we could provide water to pt, the pt began vomiting. Pt was placed on his side and paramedics were called. Pt remained in a laying position until emt's arrived. Pt was examined and released. Pt was encouraged to follow up with his pcp.

Patient became pale, had chest pains, difficulty breathing, vomited, red spot formed on tip of tongue. 911 Was called. Patient was helped to the floor and a he was no longer vomiting. He was feeling better with chest pains relieved and no difficulty breathing. The red spot remained on his tongue and he was very pale. Paramedics came and he went to the hospital.

The patient reported SOB, chest tightness, sweaty, nausea, dizziness. The symptoms were reported 30mins after vaccine administration. We checked blood pressure 151/91 and pulse <100. The EMT checked vitals again and everything was in range, The patient went home with mother and she will take him to hospital if needed.

HISTORY OF PRESENT ILLNESS: This is a 20-year-old male with past medical history of asthma, who was brought to ED by ambulance for nausea, vomiting, generalized weakness, and fever. The patient received his second dose of Moderna vaccine 10 days ago. The patient is experiencing symptoms of nausea, vomiting, fatigue, poor p.o. intake for the last few days. He was at ED 4 days ago and had a negative workup. The patient had a fever with T-max of 103 at home. He also had 2 episodes of emesis in the last few days. He denies any diarrhea, chest pain, abdominal pain. He complains of headache. He denies any blurred vision or neck stiffness. At hospital, the patient was given 4 L of fluid and had multiple blood tests. Even echocardiograms were unremarkable. The patient is unable to ambulate because of generalized weakness and dizziness.

Initial symptoms at 12hr post-vaccine typical chills & aches & nausea.; duration 6 hr. At 24hr mark, chest pains started with palpitations. Worse with exertion. Subsided and returned. Sought treatment at 48hr post-vaccine. Clear ekg, chest xray, and chest ct. Aspirin 325mg calmed the symptoms.

I had no symptoms after receiving the first dose, but after receiving the second dose I developed a high fever of 103 degrees as well as body aches, hypersensitivity, dizziness, migraines, and diarrhea. I eventually began taking ibuprofen to manage the symptoms, starting on night one. I then usually took ibuprofen twice a day after that. My symptoms were completely treated by ibuprofen and nearly all of them disappeared when I took it, besides the diarrhea. Oddly I also developed cold sweats when taking ibuprofen, which would dissipate after the ibuprofen stopped working. The symptoms kept coming back, as far as a week later, however, I didn?t have to take as much ibuprofen as before because they were becoming intermittent. Today, I almost got through the whole day without a return of any of my symptoms, until about 11:00 PM, after which all symptoms came back, the difference being that my fever is now low grade. This has been the case for two days now, no symptoms until night, when they all return promptly.

I IMMEDIATELY ASKED HIM TO SIT BACK DOWN, AND CAME OUT TO HIS SIDE. HE THEN TOLD ME THAT HIS DIZZINESS SYMPTOM INTENSIFIED. HIS BODY SUDDENLY SHOOK A BIT, I HELD ON TO HIM TO KEEP HIM STEADY. THEN I HAD HIM SAT DOWN ON THE FLOOR, THEN HE TOLD ME HE NEED TO LIE DOWN. SO I HELP HIM LAY DOWN. HIS SKIN STARTED TO FEEL VERY DAMP. HE SAID HE FELT EXTREMELY NAUSEOUS. I HAD MY TECHNICIAN CALL 911 FOR HIM, I REMAINED WITH HIM BY HIS SIDE THE WHOLE TIME UNTIL PARAMEDIC GOT TO US IN 8 MINUTES. THEY TOOK HIS VITAL SIGNS. EVERYTHING CHECKED OUT. HE DIDNOT GO TO THE HOSPITAL. PARAMEDICS LEFT. HIS MOM CAME TO PICK HIM UP. HE FELT NAUSEOUS FOR THE NEXT 5 HOURS, THEN HE SLOWLY FEELING BETTER.

Pt presented with fever, chills, severe abdominal pain, headache, photophobia and nausea a few hours after the second dose of the vaccine was administered. Few days later, pt was hospitalized comatose, and required intubation. Spinal tap showed lymphocytic pleocytosis but all viral and bacterial testing were normal. The patient said he is feeling nauseous and he said he cannot see after some seconds and also felt dizzy. we called 911 meanwhile the patient said after 5-10 minutes he can see and he still feel nauseous. He didn't go to the hospital with the paramedics. After paramedics left he said he felt fine.

He had the vaccine on 05/12/2021. It started at 1 am, he had a pretty normal night, he went to bed and woke up vomiting, that continued through the night, the vomiting continued, he had a head ache, he called me and told me that his stomach really hurt, I told him to check his blood sugar, he also had hot flashes, chills, and body aches, but mainly he had vomiting, then the next day 5/14/2021 he was getting more sleepy and less responsive, his friends tried to give him water and different things, his blood sugar went up, his friends took him that afternoon to the ER because he started not responding to me, his friends would tell me he was sleeping, so they took him to the ER, they found that he was very dehydrated, they were giving him fluids and did blood work, he was very unresponsive when he arrived, he could not answer his questions, he had a friend that was there that told them he had the vaccine, the blood work said he had a very high white blood cell count they were worried he had sepsis, they found his kidneys were not working ok, because his levels were very crazy and chest pain and severe stomach pain, and he had pancreatitis, they ruled out a heart attack, he then was admitted because he was in DKA, diabetic keto acidosis, like a result of his type 1 diabetes that got flared up because of all his reactions. They admitted him to the ICU and he was there until 05/16/2021 and basically they were just giving him fluids, potassium and other things to help his body calm down and stabilize his blood sugar. He was released and he still continues that his heart hurts but doctors say that these things resolve after a while.

Vaccine was mixed/prepared/given proper protocol. Patient sat down after vaccination and within 5-10 minutes he fainted face forward from a seated position. He regained consciousness within 30 seconds of fainting. He then vomited within minutes after he sat up after fainting. A nurse practitioner was in store shopping and stopped by to read his blood pressure (135/76) then the EMT arrived within a minute of the blood pressure reading.

Symptoms first started 3 weeks after the first injection. Symptoms are best described as mild-moderate and constant lightheadedness/vertigo/dizziness. They came on slowly the first week and escalated that by the end of the second week to the point where I went to the emergency room. EKG and all bloodwork came back normal, I have prescribed meclizine which had no effect. Symptoms vary slowly improved over the course of the next 2 months, at which point progress stagnated. Still feel light-headed constantly, have difficulty focusing, and have felt nauseous in the past, but that has improved. Made worse by caffeine and alcohol. Benadryl and Sudafed seem to have helped in the past, but only for a few days before their effects wear off.

Moderna COVID-19 Vaccine EUA I received the first dose of my vaccine at about 8:30am on January 12. Around 6pm on January 12 I started to experience chills. As the night went on the chills got worse and I developed a terrible throbbing headache. In addition to that I was feeling hot and sweating profusely. I fell asleep around 10pm and woke up around 11pm, at this time the headache was worse, the chills were getting better, and I was still hot and drenched in sweat. I was able to fall back to sleep shortly after. I then woke up at 1am on the 13th, I still had a headache, the chills were gone, I was still hot and sweaty. At this time I also started vomiting. I preceded to vomit until there was nothing left in me and went back to bed. I woke up at 3am and proceeded to dry heave as there was nothing left to come up. My headache was extremely bad at this point and I was still hot and sweaty. I could not keep down Tylenol so I used a cold wet rag to help with the headache. I was finally able to sleep again. I woke up at 5am still with a headache and still hot at this time I was able to take my temperature and it was 99.8. I fell asleep again and woke up at 9am with a temperature of 100.1, at this time I was able to take Tylenol and keep it down which broke my fever and alleviated the headache. Now at about 1pm on the 13th I am still only able to sip liquids and my stomach is upset. This was the sickest I have been in a long time. I have spoken on the phone with a triage nurse at my doctors office and I am waiting for her to consult with a doctor and call me back.

per report via email - patient receive COVID vaccine during staff clinic in the AM. During the evening patient started to feel sick. symptoms included: nausea/vomiting, uncontrollable jerk like seizures, very disoriented, respirations ragged and rapid. Family called 911 taken via ambulance to local ER. per report patient received IV fluids, medicine to treat nausea/vomiting and meds to "get his respirations under control". patient sent home and is now resting.

Fever, chills/shivering, headache, nausea, difficulty breathing, myalgia, weakness, restlessness Pain at injection site beginning two hours after injection. All other symptoms experienced during the evening, night, and morning after injection. Sudden onset, lasting 12 hours. Constant. Acetaminophen and ice used to alleviate symptoms.

Patient was seen in my office on 1/15/21 due to a reaction from the covid. Reports that "3 of the reserve doctors were running a covid vaccine clinic and were giving the injections themselves." He reports that he "received the first dose/shot in his left arm then about an hour later received the second dose/shot in his right arm." This happened on Saturday 1/9/21 in the evening and he reports his symptoms started late Sunday night/early Monday morning and were significant by later in the day Monday 1/11/21; he reports he had severe headaches, dizziness, nausea/vomiting, numbness and tingling in both arms from injections sites to forearms, "felt like he was going to die." He is unsure if he had a fever because he did not check it. He reports his symptoms improved on Wednesday and "were pretty much gone by Thursday." At his appointment on with me on Friday 1/15/21 he had no symptoms and his physical exam was normal. No injection site irritation and normal neuro exam. He reports he "notified the reserve doctors about his reaction and symptoms and was told at that time that he was given the vaccine incorrectly because both doses were given at once instead of a few weeks apart." He reports his comrades that also had the vaccine given incorrectly on Saturday "were also really sick" but states that "by Sunday the doctors realized their mistake and were giving the doses correctly just one at a time."

Immediate severe headache (12/30/20). Following day (12/31/20) persisting headache, marked diarrhea, severe chills. Went to local ER: HR 135, afebrile. WBC 12.3 w/10.4 neuts. Chem profile, EKG, UA, CXR all okay. Better with IV's. Advised not to have second dose. Doing well now.

pt presents with mid epigastric chest burning x1 hour after breakfast, 21 yo M w/ no known chronic medical conditions, presents w/ 2 hours MEG/central chest burning. Initial a/w mild SOB and LH; by time of my eval, LH/SOB had resolved and CP was 4/10. Pts sx completely resolved w/ GI cocktail. No exertional sx. No recent infectious sx. VS w/o acute abn. Well-appearing, NAD. Normal CV exam; no edema. LCTAB. PT transferred for troponemia.

c/o subjective fever with dizziness and sweating. Temperature, tmax 100.0F (A) at home. Pt received 2nd dose of COVID Vaccine at 0930 02Feb2021. Patient is a 21 year old male who presents to ED with subjective fever, dizziness, sweating and papitation 8 hours after receiving his second Moderna vaccine. He denies any chest pain, no shortness of breath. He doe shave some nausea, but it able to drink fluids well. No other significant PMH.

At 2230 I experienced severe chills, headache of 7/10 on right temporal lobe, nausea, and a fever that started at 102.3 that elevated to 103.8 At 2415 I went to VA. I had uncontrollable vomiting, dehydration, and fever. I was given 2L of iv saline and iv Zofran and oral ibuprofen. After 4 hours my vitals stabilized. Initial vitals of pulse in 130s, 18 respiration?s, and systolic bp of 130. My normal systolic bp is in the low 110s. Can contact riverside for labs and vitals and other information.

Patient had 1st COVID vaccine on 01-22-21. Had 48 hrs of typical reactogenicity symptoms including low fever, fatigue, dizziness. Felt well for post vaccination days 3-5. Then on post-vaccination day 6, he developed fevers, sore throat. Symptoms persisted x10 days and sore throat pain led to decreased appetite and 10lb weight loss. Due to fever, he was tested for COVID by PCR on 02-01-21 and was positive. He had no anosmia and Respiratory Pathogen Panel on 02-01-21 was negative for influenza and 17 other viral pathogens including negative testing to Coronavirus 229e, OC43, N163, and HKU1.

Complain of not feeling well directly after vaccination. Became pale in face and hands. Vomited. Vital signs within normal range. No other complaints. Monitored for 30 minutes. Color returned and patient felt better. Left with father under own accord.

About 4 hours after getting vaccine, I started to get a headache and my injection arm started hurting. Then a couple hours after, my skin got sensitive and burning. My muscles started hurting. 10 or 11 hours after getting the vaccine, I got severe chills; migraine; fever; nausea; dizziness. Symptoms lasted through the night. Fever went away 24 hours after receiving vaccine. 14 hours after getting vaccine, headache, arm ache, and body soreness remains.

Pt received vaccine @ 12:15 PM. Prior to vaccine pt warned nurse of Hx w/ passing out after injection/blood draw. Nurse administered vaccine and conversed w/ pt in an attempt to distract pt. @ about 12:30 PM pt c/o nausea & lightheadedness . Pt was laid down in trendelenburg position. Pt was still conscious and able to speak. Two sets of vitals were taken. Both were WNLs. Pt was observed for 30 mins and d/c home. No further medical f/u needed. - Healthcare Provider.

Started with pain in the abdomen then spread upwards into chest. Serve pain around ribs making it hard to breathe. Vomited and symptoms went away for ten minutes. Pain came back gradually in same spots. This was cyclical occurance and then just stopped.

few hours later patient had severe dizziness and nausea and had to leave work. called today (3/7) and still has both with achiness and fever

I woke up 3 days prior to my vaccination body extremely aching like deep to my bones, i have an insane headache, and ive been having the feeling of nausea all day and have been vomiting but no Fever.

Patient received 1st dose 3/5/21 @15:33 in left deltoid. Reports diarrhea starting within 2 hours on same date that self resolved but reoccurred 3/18 and continues intermittently to date of report. Also reports vomiting beginning 3/9 and lasting through 3/13 but self resolved. Also reports fatigue beginning 3/7 that continues to date of report. Also reports rash at injection site beginning Day 10 post injection that lasted for 2 days. Also reports soreness at injection site from 3/6 - 3/7. Patient denies fever, chills and headache.

low-grade fever, mild headache, significant joint and muscle pain, extreme fatigue, brain fog, nausea, chills.

8:00 woke up very tired and decided to stay in bed and rest. 11:00 When woke up again I felt sick to the stomach and had the urge to throw up. While rushing to the bathroom I lost consciousness and fainted. Hit my face against the tub and the wall. My nose was bleeding and went to the emergency room. 11:45 in the emergency room they took blood samples, a head scan and put me on an I.V. for 45 minutes.

Headaches, nausea, dizziness. Treatment was rest and otc pain medication (Ibuprofen). Symptoms we mostly abated by Sunday 4/4 and were not present morning of Monday 4/5. Dizziness returned Monday 10:30 and Nausea 13:30

Patient stated he started to feel unwell a few minutes after the vaccine, then started to feel light headed, nauseous, dizzy. He reported he felt like he was about to pass out, he felt he had dry mouth and was thirsty. To provider he appeared pale and sweating. He was instructed to sit in chair and lean forward with head down towards knees. He was provided water and his blood pressure was taken. The first reading was 89/66 pulse 74. He stated he was not sure what his normal pressure was, but was usually low. He also stated that he had a light breakfast, and didn't really drink much that day. After his pressure was taken he reported to feel slightly better, and was observed for another 25-30 minutes. After this time passed, his blood pressure was taken again, and reading was 102/74 and his color appeared better, and was more alert. After a few more minutes, he said he felt much better. He also had eaten crackers at this time. He was provided v-safe form again, and encouraged to sign up for follow-up, and to continue to monitor closely any symptoms for at least the next 6 hours.

APPROXIMATELY 10 MINUTES AFTER RECEIVING THE MODERNA COVID-19 VACCINE PT COMPLAINED OF NAUSEA, BLURRED VISION, LIGHTHEADEDNESS. SYMPTOMS PERSISTED FOR 2 HOURS.

Severe nausea. Unable to keep down ANY liquids including water for the first 24 hours after the vaccine was administered. Uncontrollable sweating (no fever), muscle spams near vaccine site on left arm and in left shoulder blade. Headache.

Became light-headed, dizzy, nausea and sweating profusely after the vaccination. This occurred within minutes after receiving the vaccine.

Patient stayed in pharmacy area for 15 minute observation time. Was released from pharmacy in good condition and left to shop with parents and sister. Approximately 5 minutes later, sister came and said he was very dizzy and sitting in chair in store. Pharmacist checked BP 131/75 HR 97. Patient had not eaten breakfast, was dizzy and shaking. Parents had gotten him a sandwich and orange juice was added in case due to blood sugar drop. Parents stated he was not presenting as having a seizure. He began feeling nauseous and vomited 3 times while waiting for EMTs to arrive. EMTs checked him out, blood sugar 93. They took him to ER to be examined and was released to go home later same day. Per conversation with parent that evening, he was doing better after having several episodes of vomiting at hospital.

pt was siting in the post shot area and began to have Urticaria and pruritus symptoms, about 15 min after shot, after another 15 min pt stated he had nausea and a headache, pt sated that he is not doing well. EMS called and pt PCM arrived pt refused trasport by EMS and left with his PCM.

At the pre-counsel stage, the patient said that he was a little nervous about getting this immunization, but wanted to go ahead and do it. I counseled him on the immunization and sent him into the booth to get the shot. Technician administered the vaccine and then told Technician that he felt a little nauseous. Technician told him to stay seated. A few seconds later the patient passed out and started slipping down the chair. Technician grabbed him from behind and called me over to assist. I yelled for Staff to call 9-1-1 and a code white. I held him from the front and Technician held him from the back and we slowly lowered the patient to the floor. Technician sat behind the patient to hold him up while I assessed the patient and tried see if he was responsive. The patient was breathing and had a pulse, so I patted him on the face and chest to see if he would become responsive. After about 15 seconds, the patient became responsive and confused. He asked what happened and if he got the shot yet or not. After talking to the patient for a moment he said he was fine, but needed to vomit. I gave him a trash can and he vomited. We gave the patient some water and sat with him on the ground for a few minutes until the paramedics arrived. The paramedics took the patient's vitals and the patient said he felt fine and declined going to the hospital. We had the patient sit in the observation area for another 30 minutes. He said he felt totally fine and was ready to go home.

Patient presented to Health Center after receiving COVID-19 vaccine at Health Center, and going back to work site. He stated when he got to work, he began feeling hot, dizzy, nauseous, and pruritic. Vitals stable. He denied chest pain, difficulty breathing. Pt given IM Benadryl 12.5 mg in office and oral hydration w/ water. He was monitored for about 30 minutes, and said pruritus had resolved, but he felt tired. EMS called to transport pt to ED. Pt called by Health Center staff about 2 hours later to see how he was doing. He stated he was on his way home as he had been discharged from ED and told to monitor symptoms.

Patient received vaccine in left deltoid and immediately responded which looked like a short seizure, losing consciousness shortly, then upon shaking responded by waking back up. Within the next few minutes the patient did the exact same thing and again responded after shaking by waking back up. Patient felt hot, weak and nauseous for quite a long time, he eventually vomited and thought he felt a little better. We called EMS and they responded by checking him over (B.S. 79 and Blood Pressure initially 87/60 but a repeat reading was 105/74 and patient felt well enough to decline a trip to the hospital. Patient was still feeling weak, so we observed him for over 2 hours and he took some water and drank some gatorade. After a bit we had him call his mother to drive him home. Patient was standing when we had one of our interns take him to his mother's car.

Vaccine was administered at approximately 6:15PM and patient was instructed to sit in waiting room for 10 to 15 minutes to make sure he was OK. He looked fine and sat looking at his phone for approximately 5 minutes. He then stood and walked to pick up window and asked if it was normal to have stomach pains. I said no and asked if he felt dizzy. He said "no" then turned to go back to his chair. As soon as he turned he said "now I feel dizzy". I told him to return to his chair and he suddenly crumbled to the floor. I had my technician call 911. After 1 to 2 seconds of being unresponsive he suddenly came to. I had him continue laying on floor for a couple minutes and then he got up on his own and went to the chair. We gave him a cool towel for his neck and had him lean his head forward and down onto a chair placed in front on him. The paramedics came within minutes and found him to be OK. After they left he continued sitting in the chair for approximately 10 more minutes while talking on the phone (to his parents, I think). He then left stating that he was now OK and that it probably happened because he was nervous

Patient reported feeling light headed and dizzy immediately after standing up after the vaccination. After sitting back down, patient reported feeling nauseous and sweating a lot. Patient was treated with a cool wet compress to forehead and neck and offered water to drink. Patient reported that he had not eaten all day and had been feeling anxious about getting his vaccination. After about 15 minutes patient was feeling better.

Approximately 10 min after receiving the vaccine pt started feeling nauseated. Pt stood up to go to the bathroom and had a syncopal episode. Pt landed face first onto the hard floor. Pt was treated by medics on scene and was taken into a private waiting room. During further assessment pt had another syncopal episode with his hr dropping into the 40's. Pt became pale and diaphoretic. Pt transported to ER via ambulance

Patient presents to ER with chest pain, worse with deep inspiration, lying flat or leaning forward. Pain is better when sitting up straight. Vomiting x 1 episode. First Moderna shot on 4/9/21. Chest pain started on 4/11/21. On 4/14/21, pain was worse, so came to ER. Patient admitted to hospital, cardiology consult, pulmonology consult. Patient is treated with colchicine 0.6mg twice daily, NSAID and metoprolol 25mg twice daily. Follow up with cards in 2 weeks. Much improved on day 2.

Nausea, fever, body aches. Took Tylenol for body aches. Waited out the fever and nausea. By 10pm April 24, symptoms were mostly gone.

He consented to transitioning to lying down on a couch and hydrating with a water bottle. BP was within normal limits. He did not faint. After ~15 minutes of lying down he felt recovered and normal. Patient was able to walk out fine with his friend.

Received oxygen via nasal cannula + Nonrebreather, Was instructed by Dr. to visit a different ER to test for blood clots. Multiple PTSD episodes in Hospital during Hospitalization.

First nausea and headache at around 6:00pm until days later. Then shortness of breathe once around 10:30. Near 11:00pm I started getting fever, chills, trouble staying asleep for more than 30 minutes. Loss of appetite for the following days. Felt like worst case of flu ever. Symptoms stayed from that Friday up until about 6:00pm on Sunday night the 25th.

Fever started the night after I got the vaccine and continued for 2 days. My body ached and I was nauseous the whole next day as well. I took some acetaminophen and the symptoms were less severe, but did not go away until time was done

Patient received the Moderna vaccine and went to sit in the observation area. Patient began to feel dizziness and nausea shortly afterwards, assisted to stretcher to lie down by RN. Provided with ice packs, juice and a snack. Symptoms resolved after about 20 minutes and patient then stated that he felt much better and felt ready to leave clinic. Ambulating well at time of departure from clinic.

Started with nausea, he said he went to the bathroom, he came back and vomited at 3:30pm, we gave him a wet paper towel to wipe his mouth and nose. Gave him another wet paper towel to wipe his forehead and gave him a water bottle. He started to have chills, he was in a t-shirt and shorts. His girlfriend gave him her jacket and we gave him a smock to put over his legs to warm him up. 35 minutes after the vaccine (3:55pm) he said he was fine and left with his girlfriend and another friend.

Pt experienced nausea, vomiting,, and heartburn after vaccination on 4/27. He awoke on 4/30 with chest pain, intermittent-radiating to his back at times. His troponin level was elevated, he was admitted for treatment of myopericarditis. He stayed for 7 days inpatient and was discharged home on 5/7/21.

Patient experienced syncope about 5 minutes after recieving vaccine. he fainted fell to the floor bumped his head then got up immediately later and reported to pharmacist around 7:40. Patient was asked to sit down, given water and powerade for fluids and electrolytes. after 20-30 minutes patient vomited. 911 was called immediately after. first responders came in 10 minutes around 8pm and took vital signs and questioned patient. patient refused to go with them stating he felt better. patient left pharmacy around 8:30 pm 5/12

Pain and tenderness at injection sight almost imediately. Muscle pain within first hour. Dizziness, weakness, loss of consciousness, nausea, and vomiting approximately 10 hours after injection.

This is a 21 year old male who had his first dose Moderna vaccine on 3/18/21 (lot#001B21A) and second dose on 5/9/21 (lot#024C21A). On 5/13/21, the patient presented to the emergency department with complaints of shortness of breath, chest pain, 'sharp shooting pain going up my leg/thigh,' pain 5/10, and nausea. Onset on 5/12/21. The patient has a history of a recent ED admission on 4/24/21 for abdominal pain, felt to be related to a recent diagnosis of hepatic stenosis, biliary sludge (unrelated to the 5/9/21 ED visit, however notable; discharged to home to f/u with established surgeon). The patient stated that this may be related to chronic anxiety. ED provider determined that acute chest pain was likely due to anxiety and/or a vaccine reaction. The patient was discharged to home with instructions to f/u if needed. No additional chart documentation available since discharge.

Patient developed symptoms of diarrhea, nausea, vomiting, headaches on 05/05/2021. Seen in the clinic on 05/12/2021 declined COVID testing. 05/17/2021 reported chills and bodyaches, positive for COVID.

Systemic: Fainting / Unresponsive-Medium, Additional Details: patient passed out from chair about 5 minutes after dose given. I was helping an intern get her 1-9 information into the ipad. There was another vaccine patient waiting in a chair, she immediately came and got me, I rushed to the patient to bring him back to consciousness. I shock his shoulders and he started to come back. Staff team members were extremely helpful. They came right away with water and help to assist with anything the patient needed. Patient stated that he felt hungry.

Pt stated that he noticed that his throat felt tight and difficult to swallow. He also stated that he was dizzy. We administered 25mg of diphenhydramine orally and monitored until a family member came to get him. He went home and I followed up with him later and he said that his symptoms had resolved

fevers, chills, HA, CP, vomiting x2, ED presention 4/22. Tx with ibuprofen, colchicine, famotidine, metoprolol. Discharged on colchicine/ibuprofen and metoprolol. Resolution of sx over next few days.

Two days after administration of first COVID-19 vaccination, patient developed blood streaked diarrhea and abdominal pain. Patient was referred to emergency room, prescribed Lomotil, workup for IBD. History of poor weight gain, C.diff infection 2 years ago (2019). Patient referred to GI for further work up.

Patient started feeling really nauseous after receiving the vaccine and had to lay down for about 15 minutes and drink some water. Patient did not need any medical attention. Patient had to call her husband to come and get her. Patient attributed it to her fear of needles.

pt received #1 Moderna vaccine yesterday, pain on left arm, shoulder started around 3 pm, c/o dizziness and nausea*1, denies any blood in the emesis, denies any traumatic injury, last OTC Tyrenol 2 tabs taken at 2200 last night without any relief,

Chest pains (tension and pressure), nausea, loss of appetite, fatigue, shart left arm pain non muscular lasting for weeks.

About an hour after getting the shot, the skin on my right arm was flushing before going numb. Then my shoulder went partially numb and it's still numb/frozen as of 2:20 am 26/June/2021. Over the course of nine hours from when I got the shot, starting at around 1 hour after, I started to get symptoms, which as of right now are still here. After my skin started acting up, my blood pressure dropped and my heart was pounding extremely hard in my chest and felt tight. My muscles have been going between losing muscle tone and being locked up extremely hard. I've been dizzy since about 3:00 pm 25/June/21, and it has progressively gotten worse to where I feel like I am going to pass out if I stand up. I have chills that make my chest tighten up and cause angina. I have been feeling very faint since around 9:00pm. I've had a mild headache since around 2:30pm, and have been feeling like I am falling since about 1am. Breathing takes manual effort. I have been feeling dissociated and nauseous since about 6pm and cannot remember much from after 6pm. I am not sure whether it is a medical emergency or not but I can't afford to go to the er.

The patient received his second shot of Moderna vaccine on 6/14/21. That night he felt ill with headaches, fever and nausea. On Friday morning (6/18/21), the patient developed a 15-minute episode of chest discomfort. He described it as a moderate midsternal chest soreness radiating to his left arm at rest and worse with a deep breath, not positional. That same evening, the patient had an even worse episode of the same chest discomfort prompting his trip to the emergency department. The patient was admitted to the hospital with an elevated troponin level. He was started on colchicine and ibuprofen.

received Moderna COVID-19 vaccine on 6/28 and since 6/29 has had worsening chest pain with shortness of breath, but it was most notable yesterday. He went to urgent care , and had an EKG, with the results being abnormal. He reports it feels better when laying down and is exertional. He had chills, abdominal pain and myalgias, headache.. impression is myocarditis likely related to the vaccine.

Pt denies runny nose, overt fever, cough, sore throat, vomit, diarrhea, rash, sick contact, visit to Area II or contact with COVID-19 hotspot/cases in last 2 weeks and no co-morbidities.

"Moderna COVID-19 Vaccine " pt received vaccine to his left arm. 10 min later (1335) he reported feeling dizzy. allergy tech and i evaluated pt. pt clammy with sweat noted to the back of his neck. pt speaking in full complete sentences. no increased WOB. left radial pulse regular and not elevated, not thready. no hives found. denied itchiness. asked pt if he felt anything stuck in his throat, he swallowed a few times and said "yes". asked if he had much water today, he said no. pt reported it felt a little hard to breathe in his throat. i had him remove his mask. pt reported nausea. denied abdominal pain. had him take a few sips of water. reported he no longer felt anything stuck in his throat, no longer hard to breathe in his throat. pt reported tingling in his left arm. pt worked the night shift. went to work at 1600 on 2/11. ate at 2000, beef and potatoes. had snacks. ate chicken and rice at 0300. usually has a full gallon of water on shift but only had a half gallon. got off at 0400. went to sleep at 0530. usually wakes at 1445-1500 but woke up earlier for this appt. appt was at 1330 but he was here early. had a protein shake 30 min before arrival but has not had much water. thinking the protein shake was causing his symptoms. denies working out today, denied energy drink intake, denied any coffee. pt took a large amount of water and then threw up. pt reported he felt much better. nausea resolved. still no itchiness. no hives. no clamminess. no trouble breathing. no globus. no dizziness. no tingling in his left arm. resolved by 1340. held Epi given he reported complete resolution of his symptoms. seemed likely the combination of being underhydrated, having slept less than usual, facility being a little warm and him wearing a mask and possibly his protein shake contributed. had patient wait 30 minutes (1410). allergy tech sat with patient who continued to speak in full sentences, tell stories, etc. had him stand up slowly. denied dizziness. still no hives, no itchiness, no globus, no trouble breathing, no nausea, no abdominal pain. advised pt of reasons to call 911 (he lives on base where we have ambulance services). advised i would be placing a VAERS report.

Patient developed severe abdominal pain Sunday night and was admitted to the hospital for treatment of acute pancreatitis.

Patient reported "feeling sick", and reported feeling dizzy, nauseous, chest tightness, and chills. Patient temperature was 100.1. Patient accepted PRN medications and inhaler.

1223 notified of occurrence after vaccination time 1219. Recipient of Moderna vaccine to right deltoid. Noted to be diaphoretic, stated had dizziness with nausea which subsequently resulted in vomiting x2. VS: 98%, 58, 102/44. Stated that he had only 2 hours of sleep today, started work at 0230 am. 1232: Stated feeling less nausea, continued to observe. VS 99%, 67, 106/58. 1243: 98%, 69, 16. Stated feeling much better, no longer has dizziness, no nausea, no diaphoresis. 1253: 99%, 63, 108/54, stated continues to feel better, no nausea, no dizziness and would like to leave, left on own recognizance.

No reaction occurred from time of administration (0830) to 1600. Intense emesis and stomach pain then began. Patient self-administered 100mg IM hydrocortisone. Blood pressure monitored via home monitor quickly decreased and heartrate increased. At 2000 on 3/2 EMS was engaged and brought patient to Hospital Emergency Department. At emergency room patient was diagnosed with acute adrenal crisis and given multiple doses of hydrocortisone 50mg and promethazine 12.5mg. At least three 1000ml NS boluses were provided in ED. Blood pressure continued to drop; lowest noted was low 80s/30s. Admitted to cardiology unit for continued observation and steroid/antiemetic support. Patient remained admitted 3/3 and was discharged 3/4 following adequate fluid resuscitation and transition to PO steroids.

Patient was sat in the front of the pharmacy to wait 15 min after vaccination. Patient agent with him came to the counter and stated patient was not feeling well. Patient was pale, sweating, and feeling nauseous. After 3 minutes (approximately 8 minutes after vaccination), patient started to vomit on himself and was given a basket. Gave Ice pack to place on neck, shoulder, and head and gave cold wet cloth to clean up vomit. Proceded to make patient wait an additional 15 minutes and patient started to feel better and was given okay to leave.

Approximately 5-8 minutes after receiving the dose, the patient became diophoretic and nauseous. Patient did vomit a dark colored small amount of emesis. His symptoms resolved within 10 minutes.

Starting on December 24 and lasting until March 1, I experienced the worst insomnia I had ever experienced, as well as depression, anxiety, and paranoia. Previously, I was doing well. My parents and I saw a night and day difference. I was also throwing up regularly. I also became much more angry and moody. I often felt sick to my stomach.

Pt received the first of the two-series COVID vaccination at 1512. Once in observation area, pt became unresponsive shortly after sitting down. This provider was called over by staff to pt, which was noted to have become unresponsive while sitting in the chair. Pt remained in the chair, did not fall or injure self. Upon arrival to patient, he was noted to have severe diaphoresis, prominent pallor, with shallow, slow breathing, no cyanosis noted. His head initially fell back and head was aligned/adjusted to maintain an open airway. This provider was speaking to the pt and performed a sternal rub to elicit a response, pt remained unresponsive and with eyes closed. Pt remained in this state for approximately 10 seconds. Pt noted to suddenly have some muscular rigidity to arms and torso and in order to prevent injury upon possible onset of seizure activity, pt was lowered to the floor by this provider and RN . Once on the ground the pt was relaxed and opened his eyes. Pt was immediately verbally responsive and asked if he passed out. Pt was able to sit up and was assisted into the wheelchair. Pt moved to treatment area where vitals and blood glucose were obtained. (@1525 BG- 104 mg/dL, VS BP 116/72, HR bradycardic @51 bpm, O2 sat 95%). Patient responses/speech was clear and pt was oriented to self, place, time and situation. Pt stated that he felt nauseated upon sitting in the observation area and admitted that he only had a coffee today, nothing to eat all day and attributed this as the cause of his reaction. Pt verbalized that he would not respond to the hospital with the paramedics and was encouraged to at least call someone to take him to the hospital. This provider spoke with the patient?s mother and explained the events. The patient?s mother stated that the patient had febrile seizures when approximately 4 years old, associated with asthma exacerbations; but pt was evaluated with a neurologist and there were no abnormal findings. Pt admitted that he has a similar syncopal episode after washing a laceration and seeing his finger bleeding. Pt states this was a few years ago and witnessed by his father that told the patient he initially fainted and then became momentarily rigid before responding appropriately (similar to today?s events). Ambulance - arrival @1536 report given and pt stated he was feeling better and declined further evaluation in truck and also declined transport to ED ( departed at 1545). Pt?s overall condition, appearance, including color was much improved. Pt denied any allergies or medication history. Past medical hx of febrile seizures at approximately age three and four, childhood asthma and one syncopal episode as an adult. Pt girlfriend arrival and she agreed to drive patient from the vaccine clinic. Pt and girlfriend were both provided with an explanation of the many concerning causes of syncope and the necessity for further evaluation in the Emergency department to rule out any underlying conditions. Both patient and girlfriend verbalized understanding and although they still declined to respond to ED directly from the clinic, both did agree to respond to ED if any changes in mental status occurred. Pt lives alone, but agreed to stay with parents and family in order to have others watch him for any changes. Pt was advised to followup with PCP and made aware that an provider would contact him tomorrow to check on his status, understanding verbalized and agreed with plan of care. Pt in no acute distress, awake, alert and oriented. Color within normal limits for and departed facility with girlfriend at 15:56, steady gait noted. Patient?s appointment for second dose was cancelled and will need to obtain clearance prior to second vaccine dose.

Patient with severe dizziness and intractable nausea/vomiting. Upon admission to the ED last night his platelet count was 9. He has a subconjunctival hemorrhage OS and frank hematuria, besides the epistaxis. Pinpoint erythematous lesions noted on his forearms. No other bleeding noted. ADAMTS13 level confirms TTP . Patient now on day 4 of PLEX treatment.

22 Y male with past medical history of environmental allergies presents with a chief complaint of abdominal pain and nausea/ vomiting. He states that on 4/7 he had some nausea and one episode of non bloody nonbilious emesis that morning. He had previously received his first dose of the Moderna COVID vaccine on 4/6. He states this morning he awoke and had significant nausea and abdominal pain and then had approximately 10 episodes of non bloody emesis that eventually became bilious. He did have some diaphoresis. He states that he eventually did have some chest pain that he attributed to all of the vomiting. He went to urgent care and did receive Zofran but this did not alleviate his symptoms. He denies shortness of breath, cough, fever, chills, diarrhea, melena, hematochezia, genitourinary symptoms, or other pain. He states that he has been having a few episodes of loose stools, but did not have any bowel movements today. In urgent care CT was performed and was suspicious for pneumomediastinum within the partially imaged lower chest and a CT esophagram was recommended. Patient presented here for CT esophagram. He is admitted for further treatment, and required a three day admission with IVF, and IV narcotics and antiemetics to control his nausea and Abd symptoms. He denied dyspnea. On 4/11 the patient was tolerating a diet and stable to dc home.

102 degree fever developed Saturday night (2 days after vaccination) and has continued through Tuesday morning (5 days after vaccination), along with nausea and intermittent chills. Use of both ibuprofen and acetaminophen reduce the fever to about 100 degrees. Injection site pain began the day of vaccination but subsided after 2 days.

The patient developed a painful ulcer on his tongue, presented as a raised bump on the left side of his tongue, symptoms began approximately four days after receiving the Moderna vaccine. Was prescribed triamcinolone acetonide paste two days after onset of symptoms, after visiting a walk-in clinic because of reduced ability to eat or drink due to pain. The doctor at the clinic requested we report the symptoms due to the timing after receiving the vaccine.

I then experienced the rest of the symptoms, minus vomitting for the rest of the day.

Patient fainted shortly after receiving second dose of Moderna Covid vaccine. When I approached the patient he was laying on the ground and very pale but still breathing. I rolled him onto his back and elevated his feet and asked him to keep breathing and try to stay calm. I asked someone to grab him some water and someone else to run to the pharmacy and grab the pharmacy manager while I monitored the patient. I asked if I could help him sit up to drink some water and he accepted. After sitting up I asked if he had eaten and if I could get him something to eat. He stated he hadn't eaten all day and that he had a panic disorder and felt like he was having a panic attack. I asked if he had medicine he needed to take to help him feel better, he stated no, he needed to just wait it out. Then he told me he felt like he was going to throw up and I got him a waste basket that he threw up in right away. I asked the patient if he hit his head or if he wanted us to call an ambulance; he stated no to both questions. The pharmacy manager arrived and sat with the patient for the next half hour or more until he felt better. He stated that his dad was going to take him home now that he felt better. Some store staff had us fill out an incident report and the pharmacy manager walked the customer out to his car where he said he would call and find his dad.

A little under 48 hours after receiving second Moderna injection, I woke up with vertigo. I have never had vertigo before. The room was spinning horizontally and it would subside after sitting up for a short while. Once it subsided when I was sitting up, I laid down and the spinning began again. I got into the shower (dumb idea) and the spinning was no longer occurring, but I still felt off-balance and got a bit nauseous. It was at its worst for the hour after I woke up, but gradually has subsided since then. I did some vertigo relief maneuvers and they've helped, but I can tell I'm not 100% back to normal as of writing (11:54am). I would say I'm 90% back to normal. This could be a freak occurrence, but since I've never experienced vertigo before, I have a feeling the vaccine I received two days prior may have something to do with it.

Developed fatigue, back pain, chest pain, nausea and vomiting the morning after the vaccination. On Saturday 4/17, I awoke feeling some better. Patient received shot in Left arm and after a few minutes became to feel dizzy and warm. Patient felt nauseous. Pharmacist applied Ice pack to patient's head. Dizziness and warmness subsided after 5 minutes.

Pt felt nauseous, hot, started getting red, and sat down on floor about 5 minutes after receiving the vaccine. I was alerted and I helped him into the vaccination room to lay down on the floor with his feet propped up. His girlfriend was there to help calm him down. I offered to call 911, he refused. I grabbed a fan for him, and turned it off when he requested. He then began shivering rather uncontrollably and said his left arm (injection site) was hurting and tingling, and at one point, said his right arm was beginning to tingle as well. We had him breath and relax, he began to return to normal and he and girlfriend discussed how he must be partly having an anxiety/panic attack. He trialed standing up a few times until he felt good enough to leave the room, all in all about 20 minutes. He sat close by for additional 15 minutes of observation, at which point he left stating he felt more like himself.

It started off with me becoming really irritable over the smallest things. I started getting cold chills even though it was 70F. I had a nightmare that felt like an eternity. My dreams get warped when I'm sick. I woke up and I had a headache that was centered around several spots in my head. I couldn't get comfortable no matter what I did. It's impossible to lay down on the side you get the shot on. It hurt like... 7x more than the first one when they're administering it. The scariest symptoms though, for me, was that I couldn't catch my breath. You know when you take a hot shower and you can't really breathe 100% because of the steam? That's how I felt all day after the 2nd dose. Also, I couldn't tell if I was full or not when I ate. The only way I could tell was stomach pains. That's all that's happening/has happened to me. I do feel better alot better though two days after having the 2nd dose.

Felt lightheaded, dizziness, then leaned back passed out and was unresponsive with convulsions for 5 seconds then started responding again. Couldn't see for 15 seconds then threw up. After fluids and crackers waited for 20 minutes and then was fine.

A patient received the first dose of the Moderna Covid vaccine. After about 10 min, he suddenly got really hot and started sweating profusely. He also reported numbness in his right arm and mild difficulty breathing. He then began to vomit. One 0.3mg EpiPen was administered and 911 was called. He began feeling better immediately and left the pharmacy with paramedics.

After getting the first round of the Vaccine, I had the loss of smell and taste happen as if I had covid again, and it's lasted for a month now. It's affecting my appetite.

Digestive pains and issues, hot and cold flashes, muscle spasms. I didn?t even want to get this vaccine but my school requires it. I?m sure this will get chalked up to a coincidence when in fact every doctor and pharmacist that is misrepresenting the pros/cons of this vaccine should be held criminally and civilly liable. I?m a healthy 23 year old who had covid 4 months ago. It was easier than the flu. There?s no reason why it should be suggested let alone required to get this vaccine without proper informed consent. Systemic: Chills-Mild, Systemic: Dizziness / Lightheadness-Medium, Systemic: Flushed / Sweating-Medium, Systemic: Hypertension-Mild, Systemic: Shakiness-Mild, Additional Details: He felt lightheaded, tried to stand up and get the pharmacy staff's attention, then fell. He said he doesnt remember the fall. The pharmacy staff helped him to sit up and he asked for water. He felt sweaty and clammy, asked the pharmacist to hold his hands. The LVN came over to check on him and started taking his BP, he then felt nauseated and threw up. The pharmacy called 911, the EMT came and every thing checked out normal. The LVN called his family and he is resting in the LVN's room.

Pt.'s grandmother states that after receiving the 2nd dose of Moderna 06/16/2021, started experiencing with extreme weight lost and slight cough. Primary visit (Blood work, X-rays, Chest X-ray) showing Thyroid levels elevated. Specialist seen 06/29/2021, Ultrasound continuing current medications. Follow-Up visit scheduled for 07/16/2021. Still experiencing extreme light-headness, weakness, loss of appetite.

Since getting the vaccine, I have developed multiple strange health conditions. The first is that I have been nauseous and usually vomiting every day for at least the past 2 to 3 months. Also since getting the vaccine I have developed nerve pain and paresthesia in my hands that does not get better with rest and that I am now being treated for because it is so bad that I cannot work on my computer anymore. I also can no longer eat gluten without severe gastrointestinal distress, despite the fact that I have never had issues with gluten in the past. Finally, I have also developed two strange growths on both of my feet in the same spot on both feet. My primary care doctor thinks that they are ganglion cysts and are benign, but I thought it strange that I would develop the same thing on both of my feet in the same spot at the same time.

The day after the shot, the patient experienced severe headaches, fatigue and nausea. This lasted one day. The 2nd day after the shot patient was feeling better and went to work. While there he had one episode of fainting where he was unconscious just momentarily. Pt slumped and DID NOT hit his head. Followed up with healthcare provider. Provider felt things were good and told pt to return if anything changed.

Patient made statements that he was having a harder time swallowing. Blood pressure was taken. We assessed and then he began to report difficultly breathing and a very difficult time swallowing. Blood pressure was taken and lung sounds assessed. EMS was called. Epi pen was given. Within 5 minutes he reported feeling much better and no issues with swallowing or breathing. EMS arrived on site and he was transferred to the local hospital.

Early on in the day of 1/6/21, I felt very sluggish and spaced. My boss asked me early in the day if I was okay because she said that I seemed "out of it." It wasn't to the point that I was unable to work, but it was a noticeable difference from my typical state. At 11:31 AM, I texted my wife and said "I feel gross and weird today." When she asked me how so, I replied, "I just feel run down, my neck hurts, and I can't focus." My arm was more sore than it typically is from the flu shot, but it wasn't unbearable. As the day went on, I felt the cloudiness in my head start to dissipate a bit and I started to feel a little better. By dinnertime, I was feeling sick to my stomach and was not able to eat very much. I chose to head to bed around 9 because I was feeling pretty ill (my neck had really started feeling sore again and my stomach was very upset.) At 1:30 AM on 1/8/21, I woke up with a horrible headache. I get migraines from time to time, but this was unlike anything I had ever felt before. It felt like my head was in a vise and I was very close to asking my wife to take me to the ER. I never go to the doctor, but it legitimately scared me. My neck was extremely sore and my head hurt so bad that even laying it on a pillow hurt. I was extremely nauseous as well. I took a few extra strength Tylenol and laid my neck on a heating pad with an ice back on my forehead. The sound of our humidifier was too much for me, so I also out in earplugs. After an hour or so, I fell back asleep. When I woke up this morning, I still had a headache and I took an Excedrin. To this point, I have felt a little sick to my stomach, but the intense headache has been kept at bay. My neck is still sore but it is also somewhat better.

Pt states 1 episode of vomiting today 0100 without blood, 8/10 constant bodyache. Pt denies taking medication PTA.

Contractor presented for Dose 1 of Moderna Covid Vaccine. At 1100, contractor reported headache. PMH: undiagnosed hypertension, heart palpitations. VS 1100 BP 129/67 HR j55 02sat 99% RA VS 1118 BP 138/83 HR 55 02sat 99% RA 1215 BP 148/75 HR 53 02sat 100% RA 1144 Headache resolved, reports lightheadedness 1155 Reported nausea 1215- all symptoms resolved. Contractor left observation area accompanied by his supervisor.

INITIAL TROPONIN 4.7, EKG REPEATED WITH SOME MINIMAL ST ELEVATION, 324MG ASA GIVEN. COORDINATING TRANSPORT TO ANOTHER HOSPITAL WITH CARDIOLOGY SERVICES, TROPONIN REPEATED AND FOUND TO BE 10.8 WITH GLOBAL ST ELEVATIONS ON EKG. PT TRANSPORTED TO HOSPITAL FOR PRESUMED PERICARDITIS. AWAITING UPDATES ON PT CONDITION TOMORROW.

Nothing at time of injection. In the evening, soreness in the arm (strong but on par for what I would expect). Next morning, I starting feeling sick, with flu symptoms. Aching muscles, sensitive skin, mild headache, sore eyes, and in the evening, nausea. Temperature gradually rose. ~99.7 at 6pm, 100.1 at 7pm, 100.7 at 8pm, 101.7 at 9:30pm. I took some Tylenol around 7pm, and then some theraflu at 10pm. Fell asleep at 11pm. Periodically waking up every 30min from 1am onwards. Muscle discomfort, sweating, and nausea continued until 3:45 AM, at which point I was feeling better and was able to sleep continuously. Feeling much better in the morning on March 4th. Temperature was 99.9, then rose to 100.0 at 11am. Now, at 3pm, feeling fatigue, exhaustion. No headache. Normal sense of taste and smell. No couch. Taking another day off from work. Temperature is currently 100.0. Taking more theraflu now, have not taken Tylenol today.

Patient received a COVID-19 vaccine on 04MAR21 @ 1400. At 1645, he returned to the vaccine room shaking and his eyes were tearing up. He complained of pain at the injection site, headache, nausea, abdominal pain, chills and joint pain. His temperature was 98.4 @ 1712. He also complained of increased saliva production. He laid down on a cot and an emergency rescue blanket was applied. He was reassured and monitored by our onsite RN. He was stable and left at 1755.

Symptoms: High levels of pain in the arm that received the vaccination, runny nose, stuffy nose, headache the morning following the vaccination, high fever (102-103) the night of the vaccination, and nausea the day after the vaccination.

First vaccine on 1/26/21 began taking three naps a day and arm pain. Given Ibuprofen. Seemed to resolve after 5 days, but continued to nap three times a day sometimes. No fever, but hot flashes at times. Booster given 2/23/21. Began to nap again three times a day or want to stay in his dark room. Began to have a lot of sinus drainage. Gagging at times. Pain over right eye area. Pain in right flank and stomach area. Severe headaches and possible body aches. No fever, but increased and more severe hot flashes that would wake him. 2/28 had a tonic clonic seizure after presenting with a distressed look throughout the day. Slept and upon waking began vomiting until 3am. Was at the ER and given many tests, pain meds and nausea meds. Ruled out kidney stones with CT scan and urinalysis. No fever, but hot flashing continued. ER visit lasted from 2/28/21 through 3/1/21 ER Physician suspects possible reaction to vaccine. Followed up with Neurologist and General Practitioner and both suspect reaction to vaccine.

1/19 began vomiting and was hospitalized. On 1/30 he was discharged and later received the vaccine. It begins with weakness and a lack of appetite. He started coughing up foul-smelling secretions. He makes the arrangements to take him to the emergency room again, he called 911 to be transported, when they are taking the information from the POC, he is under oxygenation to 44. They take him in the ambulance, he receives CPR, they transport him to the facility where he arrives lifeless.

Faint; Migraine; Dizzy; Disoriented; Joint pain; Fatigue; Muscle pain; Nausea; Slight fever; A spontaneous report was received from a consumer concerning a 23 year old, male patient, who experienced faint (syncope), migraine, dizziness, fatigue, disoriented (disorientation), joint pain (arthralgia), muscle pain (myalgia), nausea, and slight fever (pyrexia). The patient's medical history was not provided. Concomitant products known to have been used by the patient, within two weeks prior to the events, included allergy medication unspecified. On unspecified date, the patient received their first of two planned doses of mRNA-1273 for prophylaxis of COVID-19 infection. On 10-FEB-2021, the patient received their second of two planned doses of mRNA-1273 (Lot number: 013L20A) intramuscularly in the left arm for prophylaxis of COVID-19 infection. On 10-FEB-2021, after vaccination, the patient experienced faint, migraine, dizzy, fatigue, disoriented, joint pain, muscle pain, nausea and slight fever. Treatment for the event included Ibuprofen and Excedrin. The patient received both scheduled doses of mRNA-1273 prior to the events; therefore, action taken with the drug in response to the events is not applicable. The event of faint was considered recovered. The outcome for the events of migraine, dizziness, fatigue, disoriented (disorientation), joint pain (arthralgia), muscle pain (myalgia), nausea, and slight fever (pyrexia) was considered unknown.; Reporter's Comments: Based on the current available information and temporal association between the use of the product and the start date of the events, a causal relationship cannot be excluded.

Patient tested positive for COVID with no symptoms. Patient went to the ER for appendicitis and was tested before having an appendectomy.

pt started feeling nauseous/hot/sweaty in the after in waiting area and signaled, bench pharmacist made it to chair and guided him to floor, pt was alert and did not hit head. may have lost consciousness for a few seconds while on the floor. elevated his feet with chair. pt opened eyes and we chatted and did breathing exercises. pt states history of fainting w/ IVs and IMZs in the past. declined offer to call someone for him, stated his people were too far away. called emergency medical and they responded, and checked him over. did not check back in with pharmacy, but emergency medical released him and walked out with him.

Soreness and swelling at the injection site 2-24 hours post-injection, Chills 12 hours post-injection

The patient received his first Moderna vaccine on 4/8 at 9:45 am. At 9:55 the patient reported feeling a bit nauseous and like he "broke out in a cold sweat." Denied dizziness, lightheadedness, difficulties breathing or any other symptoms. Patient accepted to laydown and was provided water. Reported feeling better after that. VSS...BP 110/70 Pulse 65 Resp 16. Staff sat with the patient and his dad for an additional 10 minutes. Reviewed symptoms to watch for at home. Confirmed with the patient that he felt back to base line then allowed him to leave with his dad. On 4/13 a follow up call was made. Patient reports he has had no further symptoms and felt it was due to being nervous. I encourage him to inform his vaccinator when he comes in to get his second dose. Patient stated he appreciated the call and denied having any questions or concern.

I woke up in a full sweat and it was cold in the house after waking up had a massive headache with body pain was very hard to wake up to go to work when I did go to work I was in pain and very nauseous was sent home from work and go home lay down and went back to the pain in suffering woke up the next day feeling good

Upon arrival to ED, was hyperventilating and had anxiety. ED BP 121/74 HR77 RR20, T98.2F. EKG normal. Monitored in ED, then discharged once stable.

On 4/15 at 1:50pm my injection arm started getting very itchy. I have a sore located 3 inches next to the injection site. Approximately 1 minute after it began to itch I got very hot and nauseous, approximately 30 seconds later I was unconscious and beginning to seize. I was unconscious for about 1 minute. When I awoke I couldn?t see anything, my entire body was numb and in pain. I couldn?t move my fingers. I was rushed to the ER but by the time I was seen I was fully coherent and fine.

He sat at the table for observation period ?stood up and turned, stated he was feeling nauseated, took a step forward and then fainted forward onto a different table. Ice pack applied to forehead; patient sweaty but responding, stated hands tingled but no other complaint. Breathing regular, not labored. Admits he had a similar reaction to a vaccination about 3 years ago ?does not recall type of vaccine but agreed not life threatening reaction. Denied having any allergies that he is aware of. Admits he did not eat recently. Patient stated he did not hit his face when he fell onto the table, denied physical injury. Assisted to sit up while on floor for a while, then to stand and sit back onto a chair. Given bottle of water and granola bar. Orthostatic blood pressure checked at 12:25pm ?sitting blood pressure was 110/72 and standing 108/62. Patient stated feeling well; had him wait out a 30-minute observation period rather than 15-minute period. Walked out of building with staff and his sister; encouraged sister to drive them home, and for him to eat.

The patient developed fatigue, malaise, skin allodynia, fever (103.5F) and took acetaminophen and ibuprofen. The fever resolved but he was diaphoretic. He also had a headache, vomiting, then lost consciousness overnight and had a tonic-clonic seizure lasting 10 minutes. He was brought to the emergency department and had 2 more seizures there which was controlled with lorazepam. He was admitted to the hospital and started on broad spectrum antibiotics (acyclovir, ceftriaxone, and vancomycin). On 4/11/21, the patient was extubated and improved symptomatically, and antibiotics were narrowed to doxycycline. He was then discharged on doxycycline and levetiracetam.

Pt presented with sweating, nausea, muscle cramps and contractility after receiving 1st Moderna Vaccination

Felt nauseous within 5 minutes after receiving Moderna vaccine. Patient started feeling better after drinking some cold water and was stable approximately 10 minutes after. Patient was under pharmacist's observation for 30 minutes.

Fever, headache, chills, nausea, insomnia. Treatment was time, rest, hydration, and eating enough carbs.

Body aches, chills, headache, diarrhea. All started at 8:30pm on April 26, 2021. The body aches and headaches continued to the next morning

I Administered Generic epinepherine auto injector to his right thigh, and within minute or two, his breathing normalized but pt still had throat tightness. EMS arrived at that point and examined the pt and transported him to local hospital.

client notified medic he wasn't feeling well after receiving 1st dose of Moderna vaccine. he appeared pale and diaphoretic. He vomited x2. Local EMS called. he was evaluated and refused transport via EMS. his condition improved and he was released.

Individual stated he was anxious about needles and the vaccine. Within 10 minutes of receiving the vaccine, he stated he was lightheaded, fatigued and slightly nauseated. He did not faint. He was given cold water and we had him lay down. We observed him for 30 minutes after onset of symptoms. He was too fatigued to go back to work. He was escorted home. We checked on him 5 hours after the vaccine. He stated he was still very fatigued but was not lightheaded or nauseated. He was able to eat and drink.

recurrence of Ulcerative Colitis symptoms: abdominal pain. constipation, diarrhea, blood in stool. Started with constipation and has now turned to mainly abdominal pain, blood & mucus in stools.

patient started feeling light headed in the chair after the vaccination, then he started to faint and fall out of the chair. our cashier caught him, we then got him onto the ground. he was very pale, sweaty, nauseous, and dizzy. he fainted a second time while sitting on the floor. we got him a garbage can in case he got sick, and an ice pack and bottle of water to help cool him down. after 10 minutes he stated he felt much better, and he went back into the chair. we asked him to wait another 15 minutes to make sure he was okay because he had to drive himself home. after that period of time i took his blood pressure and it was normal. he said he was going to go get something to eat here in the store before driving and he would come back to let us know if he started feeling sick again.

After 2nd dose (May 3rd)...May 6th started to notice swelling in top half of both hands, stomach and chest developed hives and a rash. Not very bothersome, hands are very warm and extremely inflamed. Took allergy medications, they haven't done anything. No reactions after 1st dose, this is all after 2nd dose.

lightheadedness/ tingling sensation in head, fatigue, loose stools, sore throat, nasal congestion, dry itchy skin. all started about a week after vaccination. patient evaluated one week after symptom onset. no concerning findings

23 yo man in high school when standing, admitted wit acute onset epigastric abdominal pain after a run, found to have mild elevation in troponin, flat at 0.04, dynamic EKG changes admitted or work-up 1. Acute epigastric abdominal pain/elevated troponin: The patient was chest pain free from the time of treatment with Pepcid, magic mouthwash, and Toradol in ED. His echo is without wall motion abnormalities, troponin is flat and remains chest pain free. The patient had a CT PE protocol study and CTA coronary without acute findings. He had a cardiac MRI which showed no acute myocarditis but may have had some delayed contrast enhancement and may represent prior myocarditis. He was started on low dose lisinopril 2.5 mg po daily for cardio-protection, will reduce physical activity with slow return to activity, and follow with cardiology with an urgent exercise stress test as an outpatient. His u tox shows cannabinoids and no other intoxicants, His lipid panel wnl, HDL 65, LDL 84, His TSH is wnl. His CRP and ESR is wnl. 2. Left axillary lymphadenopathy: Discussed with patient lymph nodes seen on CT scan in left axilla and left pectoral region. Patient cofnirmed he had a COVID 19 Moderna vaccine in the left arm on 4/3/2021. Epic documentation notes in the right axilla, but patient is clear that he had the vaccine in the left arm. Axillary lymphadenopathy is not surprising in this situation. 3. Small 4 mm nodules noted on CT scan: The patient had no infectious symptoms, no cough, no fever, and no elevated WBC. He does smoke marijuana but has no tobacco use. He can follow with PCP to further risk stratify the nodules, either with PPD, and consideration of repeat scan in 6-12 months as needed. 4. Mild bilateral gynecomastia: I informed patient this has been found. He is not bothered by this finding and it seems an incidental finding. 10:25am feel nauseated Vital signs 122/82 Pulse 67 pulse ox 99%; 10:35am patient reports feeling very anxious and dizziness; 10:50am pt reports chest pain; 911 called; BP 136/86, Pulse 77, Pulse ox 98%. 10:55 am EMS arrives patient monitored, glucose 83; no abnormalities in cardiac monitoring. 11:03 released by EMS - instructed to rest and take deep breaths.

05/13/2021: Very slight sore arm, time 15:30. Very sore arm and moderately high fatigue with some faint body aches, time appx 20:00. 05/14/2021: Arm pain lessened but moderately high aches and muscle fatigue and slight nausea, appx 08:00. OTC cold and flu medicine alleviated symptoms. Light symptoms continued until appx 16:00

The evening of vaccine, patient started experiencing ringing in both ears and loss of hearing. The next day, hearing and ringing had not improved. Patient began vomiting and experienced extreme vertigo. On the third day, condition had not changed. Patient's urine is dark and he cannot keep down any food.

Arm soreness/aches started about 6hrs in (though injection sight was tender/sensitive to the touch pain became mostly localized around the armpit and shoulder area. Where the pectoral connects under the arm), then around 8hrs a headache and chills started. Chills and headache progressed and got significantly worse throughout the night. By around18hrs headache had progressed to a migraine and nausea started (likely due to the migraine). Simply sitting up in bed was enough to push nausea to vomiting at which the migraine subsided for about 30min-1hr. Chills and aching arm persisted. By 24hrs headache/migraine was nearly completely gone and chills had subsided. Arm pain persisted and got worse. Noticed around this time that there seemed to be mild swelling in the armpit. Around 48hrs in armpit area is still feels slightly swollen and causes a good bit a pain depending on how arm moves/rests. Injection sight still lightly tender. (currently the only symptom persisting)

In a few minutes after vaccine administration , pt vomited a small amount and felt hot for few minutes. pt started feeling better after few minutes and was monitored for at least for 15 minutes.

I had fatigue a few hours after second dose, after that I had dehydration symptoms such as headache, chills, body aches fever, nausea, all overnight, next morning went away by midday.

This regulatory authority case was reported by an other health care professional and describes the occurrence of APPENDICITIS (Appendicitis) and COVID-19 (COVID-19) in a 23-year-old male patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch no. 040A21A) for COVID-19 vaccination. No Medical History information was reported. On 09-Mar-2021, the patient received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) dosage was changed to 1 dosage form. On an unknown date, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 02-Apr-2021, the patient experienced APPENDICITIS (Appendicitis) (seriousness criterion medically significant) and COVID-19 (COVID-19) (seriousness criterion medically significant). The patient was treated with Surgery (appendectomy) for Appendicitis. At the time of the report, APPENDICITIS (Appendicitis) and COVID-19 (COVID-19) outcome was unknown. Shortness of Breath, fever, chills, "blacking out," difficulty breathing, dizziness, nausea, vomiting, weakness; starting after vaccination approximately 40 minutes. patient was at home at symptom onset

Systemic: Dizziness / Lightheadness-Medium, Systemic: Flushed / Sweating-Medium, Systemic: Nausea-Medium, Additional Details: about 6 minutes after 1st dose was administered, pt complianed of lightheadedness, dizziness, nausea and sweating. His BP was 113/61, pulse was 46. he lay on the floor and elevated both feet onto a chair and immediately after doing so, the symptoms began to improve. his BP was re-checked at 11.32am 132/73 pulse 58. i reached out to the patient at 12.02pm and he noted that he was not experiencing any symptoms.

Debilitating nausea for three days following injection, chronic indigestion and acid reflux over the course of the following month. No treatment yet given.

Systemic: Dizziness / Lightheadness-Severe, Systemic: Nausea-Severe, Additional Details: Pt laid flat on the back at event then recovered in about 10-15 minutes

Patient passed out in the chair (in the waiting room), while waiting the 15 minutes. He leaned over in the chair and his girlfriend noticed. He came to quite quickly. He was pale and sweating. He felt nauseated but never vomited. He stated he hadn't eaten in over 6 hours. He had been working outside in the heat and hadn't had much water, so those may have been contributing factors. He stayed in the waiting area for 30 minutes and felt completely fine when he left the store.

Was fine for ~8 hours then started to feel ?off? after completing an upper body workout. I then felt nauseous and threw up, and my body began to ache. 12 hours post vaccination I began to develop a fever of 99.8 with the use of acetaminophen and had severe chills / muscle aches. The fever progresses to 101.1 about 36hours post vaccine and declined to 100.8 48 hours post vaccine. By 72 hours the fever was gone as well as all symptoms excluding fatigue and a slight headache.

I received the vaccine on 1/7/21 and the next morning (1/8/21) I woke up with a fever of 100.5, a headache, and nausea.

One hour after injection, at 1500, i experienced high sensitivity to pain. Every inch of my limbs ached, including headaches. I took ibuprofen, but was later advised against it. The next day around 1300 i experienced chills and began shaking uncontrollably. It didn't stop for an hour, where i got incredibly nauseous and tired and slept the rest of the day. The next day, starting at 0700, i felt the need to vomit and had diarrhea. This continued until i fell asleep at 1030. Afterwards the majority of my problems passed.

17 hours after I was given the Covid-19 moderna vaccine I became very febrile, with a fever of 102.2. This was with chills, extreme fatigue, diaphoresis, nausea, and diarrhea. I realize that these are side effects, but I?m reporting it because I am not going to get the second dose. The effects of the vaccine were worse than when I had covid.

24 year old male who had his COVID vaccination 08:00 yesterday. By last night about 22:00, he start an upset stomach. He has thrown up twice. 4-5 episodes diarrhea. Body aches. No fever. Little bit of scratchy throat but no itching or difficulty swallowing. No skin rash. Left arm where he got shot is sore but is not red or swollen. He has not taken anything for this. He is concerned he is reacting to the COVID vaccination. Patient describes symptom is mild-to-moderate. But just concerned so he came in. No known infectious exposure. He is active duty and works as security at the front gate. Air Force is vaccinating all their essential personnel. Allergies/ Adverse Reactions: No Known Drug Allergies Allergy (Verified 01/15/21 03:38)

Pt became lethargic, slow to respond, speech muttered, shaking, drooping right eye, nauseous. BP 169/108, O2 sat=98%, pulse 100. Paramedics called and patient transferred to hospital.

Patient became nauseated at 11pm 1/20/21 and fever of 101, passed out in bathroom dry heaving and woke up at 4am, Heartrate 182, short of breath, continuous vomiting, called EMS, was given zofran on route to ER, upon arrival to ER heartrate reduced to 130's, patient reports being given toradol and IV Fluids, and was released. Patient reports fever range of 101-103.

Developed fever less than 24 hours after vaccination. Fever resolved on 01/21/2021 @ time of office visit. He was having symptoms at time of office visit. The symptoms were chills, headache, nausea,lack of appetite,light headiness,congestion & dizziness.

Chills starting at about 11:00 the night of receiving the vaccine, followed by intense fever and nausea. These lasted throughout the night until about 7:00 the next morning. Had an on and off fever throughout the course of the next day and extreme fatigue. By the second day after the vaccination, only fatigue and headaches persisted until about 6:00 in the evening. Then all symptoms ceased.

One week after vaccination?heavy fatigue and frequent vomiting. Fatigue seemed to subsist after the day ended, about 12 hours. Vomiting ceased. No other symptoms present.

24y/o male with remote history of Kawasaki disease fevers, chills, night sweats, anorexia, nausea, vomiting, dehydration, chest pain, dyspnea, constipation. Symptoms began around 1600 on 27JAN21 (day of immunization). Worsened on 28JAN21. On 29JAN21 presented to primary care with tachycardia prompting IV fluid resuscitation.

Severe Headache beginning 1 hour after receiving the shot. Painful nausea began 4 hours after the shot, at 5 hours began vomiting. I experienced painful nausea and severe vomiting from Sunday night at 9 pm until the last time I vomited on Wednesday night at approximately 11 pm. Nausea lasted until approximately 7 pm on Thursday. (Total of 4 days). Intermittent high temperature/fever, the highest recorded temp was 101 on the second day. Experienced severe exhaustion, spent 18-20 hours a day in bed, and was still exhausted every day until Friday. Also experienced intermittent chills and body aching until Friday. Some pain and soreness at the site of the shot as well as throughout arm but was not severe.

Dizziness, nausea, backache soon after, but went to work 1/31/21 woke up and had palpitations, dizziness, vomiting, chills, body ache, headache, runny nose intermittently all day 2/1/21 continued to have palpitations into the evening 2/2/21 no further palpitations today

Moderna covid-19 vaccine EUA. Night sweats, body aches lasting 72+ hours, body shakes, hallucinations at night, headaches, dizziness lasting 72+ hours, extreme fatigue lasting 72+ hours, sore throat, joint pain, nausea , pain at injection sight lasting 72+ hours

Nausea (lasted 24 hours, no alleviating factors) Severe Headache (lasted 12 hours, alleviated with acetominophen) Fever (peaked at 102°, lasted 12 hours, alleviated with acetaminophen)

diarrhea,bodyache,chills. Pt reports loose stools starting 2100 last night, every 30 minutes, denies blood/mucus/pus. Pt received COVAX dose two yesterday. CC: diarrhea, abdominal cramping HPI: 24yo AD M presenting to ED with 1 day of the above symptoms. Pt received dose #2 of COVax yesterday then began having multiple bouts of watery diarrhea. He has no focal abdominal pain, no emesis, is PO tolerant. He has no dyspnea, no chest pain, no URI/LRI symptoms, no subjective focal neurologic deficits.

fever, chills, headache, nausea, muscle aches, and SORE THROAT Treated with ibuprofen with mild improvement. Presented the following morning to COVID Testing Center with symptoms and was tested for COVID due to sore throat (not a "common side effect")

On 2/24/21 around 1:30 PM, Patient was not feeling well and temperature was 100.8. He was able to take Tylenol but then the staff noted his eyes rolled back in his head. 911 was called but he was not transported since his temperature was now normal and he seemed to be at baseline. He did not eat or drink much liquids later that night and in the morning of 2/25/21 around 9 AM, staff could not get him up from bed. He was incontinent and had temperature of 97.7. He was staring off and not talking or responsive. 911 was called and he was transferred to the ER and later admitted. All labs and Xrays were normal and later on 2/26/21 he was awake and alert. Of note, Patient also received news on the afternoon he got his second vaccine that a favorite staff person had been killed in a MVA. Patient was upset at this news at the time.

I did not feel anything shortly after the injection, but later in the evening, started to develop a normal post-vaccine soreness. I thought little of it and went to bed. I woke up just after 3am with chills, fever, and worsened arm pain, to the point that it restricted my arm's range of motion and speed of motion. Interestingly, the arm is not red or swollen, but is painful near the injection site, and the area around my clavicle on that side is somewhat swollen, but not red or painful (likely a lymph node). I was unable to get back to sleep due to these side effects (primarily chills and fever). After a few hours of trying to get back to sleep, I took some Tylenol and tried to eat breakfast. I was unable to keep much down, and had to arrange a day off of my internship, and was finally able to sleep on and off for the next few hours. The symptoms have somewhat lessened, but have not resolved, after about 22.5 hours post-injection. I fully expect this to resolve or mostly resolve by the 48 hour mark. I mostly found this unusual due to the severity of the reaction and after only the first dose, and wondered if my young age could have something to do with the clearly robust immune response so soon after inoculation.

Immediately after vaccination, patient felt dizzy, lightheaded, and nauseous. Immediately laid down. Observed to be pale and diaphoretic. No loss of consciousness, alert and oriented throughout. Patient reported that he has previously had fainting episodes associated with blood draws but not previously with injections. Patient was evaluated by staff and EMS. After 5-10 minutes lying down, felt better and was able to ambulate. Vaccination occurred at 1:35 pm; BP at 1:40 pm (lying down): 90/62. BP at 1:50 pm (sitting up): 110/78, HR 70, SpO2 100%. Patient felt better and refused transport to ED for further evaluation.

Before going to bed, I had chills. Throughout the night I woke up feeling extremely cold multiple times. In the morning I awoke sweating. My temperature in the morning was 98.2°. Throughout the day on 03/13/21, I was extremely sore in my hips and legs. The arm where I was administered the shot was also sore. In addition to the soreness, I felt cold throughout the day, and I had a headache until I ate around 12:00pm, but I did not have much of an appetite.

Patient was vaccinated on 14MAR21 at 2000. He returned to the vaccine room at 2130 complaining of nausea/vomiting, chills, warm face and increased heart rate (115-120). Oxygen saturation was 98%, respirations 16. Patient has a history of anxiety. Patient was evaluated and monitored by our onsite registered nurse for approximately 20-30 minutes before going home. He is to follow-up with our onsite nurse as needed.

Fever, headache, site of injection in deltoid sore, nausea, loss of appetite, weakness, lack of sleep, all within 24 hrs

I kept a log of the events that occurred. Notes: 25mg amitriptyline taken nightly for the last month, 200mg motrin I?ve never taken pain killers before this, time is in military. Mar 18 1430 (1 hour 30 mins after my shot) i have a compressing headache toward the back of my head so i take 1 motrin. 1700 i head home, take 1 amitriptyline and head straight to sleep. ( i have no apatite so no dinner ) Mar 19 0100 The pain starts, it?s unbearable. I am sweating heavily and im also shivering cold. i have a high fever I have body aches which turns my shivering into shaking which keeps my wife up all night. In my dreams i feel pain. I talked to my deceased mother, i hold my deceased cat and i relive some of my most regrettable moments. 0700 I wake up, the bed is soaking wet. I take a motrin and get dressed. it takes me an hour longer to get ready for work because, the pain has sapped my energy. My supervisor sends me home seeing how horrible I look. 0900 I take an amitriptyline and lay down. I ask me wife to close to the storm shutters to darken the room. They are normally pretty quiet to close but, this time they are deafeningly loud. I struggled to get 1 hour of sleep. 1500 I wake up due to shaking so bad that i hit my head on the wall. I then take 2 motrin and about 30 mins later i notice theres a tightness in my chest. i take 1 tums. I know that my body is exhausted from the pain and my mind is clouded but i really wanted to do all of the tasks that i was planning on doing this weekend. All of the tasks end up failing. 2100 i try the tasks again and end up failing again. 20 Mar 0100. I eat a pear and 2 cheetos. i do not take a amitriptyline and i lay down. i have no pain or fever. (Dream start) The dream i had is of me watching myself get thrown around by a ghost. The ghost possesses my body for a short time. I grab some kittens (cats are a comfort to me so im guessing that my subconscious trying to reel me back in) I then wake my sleeping wife and tell her theres a ghost and we need to leave. We walk outside and im in an old neighborhood that i grew up in, everything darkens and im in pitch black feeling my way around a room. I open my eyes and im in a hotel. The front desk staff will not let me take the kittens up to my room so i leave them. When i walk into the room, it?s full of kittens (dream end) and i wake up 21 Mar 0900 I wake up and I have a sore throat and tightness in my chest. The soreness prevents me from going back to sleep and the tightness in my chest causing some anxiety. Once I calm myself down and convince myself that im not having a heart attack I take 2 motrin and some tums and the pain goes away from the day. These could be some withdrawals from not taking amitriptyline since 19 mar in the morning. This is still happening as I write this report. (23 March 2021)

Patient had reported onset of symptoms 03MAR21 @1900 following 1st Dose of Moderna on 02MAR21. Patient reported to Emergency Department at 0000 04MAR21 with the symptoms of Vertigo, Lightheaded, Nausea, Chest Tightness, and SOB. EKG, UA, and Chest X-ray ordered by ED Provider. ED Provider notated that "CXR and ECG Normal". ED Provider diagnosis: "Acute Pleuritic Chest Pain, Dizziness, Immunization Side Effect". Patient received APAP and Ibuprofen in ED and reported that chest/pleuritic pain improved following administration but complete resolution did not occur for another 24hrs. Patient discharged from ED on 04MAR21.

10:55 - complains of dizziness and nausea Immediate assessment by onsite EMS and NP 10:57 - VS 102/63; 76;O2 sat on RA97%. Provided a fruit and cereal bar, and cool water 11:15 VS- 107/72;P 84. Felt better after eating snack and drinking water. Admitted to not having much to eat or drink prior to coming to clinic. 11:15 - discharged home. Nausea and dizziness gone.

PATIENT FELT DIZZY AND NAUSOUS AFTER VACCINATION. PATIENT SAID HE LOST COLOR IN HIS VISION. PATIENT'S HEAD ROLLED AND THEN HIS WHOLE BODY BECAME STIFF BEFORE COMING BACK TO CONCIOUSNESS. PATIENT SAYS THIS HAPPENS WITH ALL VACCINATIONS AND DRAWING BLOOD.

Patient got dose #2 of vaccine on 02APR21 at 1552. At 1740, patient complained of headache, dizziness, nausea and warm to touch. He laid down on a cot, had snacks and water. B/P 136/69, HR 70, O2 sat. 97%. He was observed by an RN for an additional 30 minutes before sitting up for 10-15 minutes at which time he stated he felt better. At 2000, he returned to the vaccine room complaining of nausea and extreme fatigue. He was given Benadryl 25 mg po. O2 sat. 98%, HR 94. Patient sent home.

Fever (up to 102F), chills, slight nausea, tension headache on upper neck, restlessness, joint pain, all beginning around 5pm. Went to sleep at 8-10pm (challenges sleeping). Woke up at 8am with 99.0F temp. Showered. Back to 97.6 by 10am.

Moderna COVID-19 Vaccine EUA Fatigue and pain at injection site starting at 8:25 PM. Fever developed over the course of the night; temperature 101.4 degrees when measured in the morning. Fever broke at around 2:00 PM. Moderate-to-severe headache, moderate joint pain, and fatigue lasting from waking at 7:15 AM until 4:30 PM. Some nausea.

Pt became nauseated and began dry-heaving within 5 minutes of vaccination. Did not vomit, but did become pale. Pt was seated when symptoms began and remained seated throughout. Applied cool, wet, paper towel to back of neck. Dry-heaves resolved after approx 1 min, and provided pt with water to sip. Pt denied any other symptoms. Recovered completely after about 10 minutes.

Became extremely exhausted by the evening with slight back pain. At about midnight, woke up with fever of 102, severe back pain, and cold sweats. Did not sleep through the night. Woke up with vomiting, severe body aches, cold sweats, shivering, continued fever of 102. Took 2 tylenol every 6 hours and fever did not drop below 100 at any time. Had severe body aches mostly in the back. Slept for a few hours at a time the day after the vaccine. Constant shivers, sweating, vomiting body aches. Took Tylenol before bed, woke up the next feeling. Second day to present some brain fog, but not other side effects continued on.

Woke up on the morning of April 10 with nausea and chills. Had a headache since the night before, but I sometimes get headaches due to neck pain so I cannot fully attribute the headache as an effect of the vaccine. Vomited three times within two hours and experienced diarrhea at the same time. Fell back asleep for four hours and felt little nausea and no chills afterwards. Took ibuprofen for headache 24 hours after time of vaccine. Felt back to normal the next morning.

Fatigue, for about Two days afterwards. Diarrhea has persisted for four days afterward. Anxiety Disorder has been acting up, this could be from anxiety over the vaccine or a side effect of the vaccine. I?ve felt more anxious since I received my vaccine, not severely, just noticing more than a typical day. I?ve only received the first dose, if it happens more after the second I will file an additional report.

Severe vomiting (at least 8 times) and diarrhea in the evening. Elevated temperature (99.2-99.5°F) with chills and other fever-like symptoms. Muscle ache around injection site, but no redness or swelling. The morning after, experienced mild fever (temperature 100.3°F) and fatigue.

Extreme fatigue beginning 5 hours after vaccination. Uncontrollable shivering 6 hours after, alternating (rapidly) with overheating and uncontrollable sweating. Eventually fell asleep ~11:00 PM. Woke up at 2:20 AM thirsty, shivering. Used restroom and stomach started to feel 'off' or 'upset,' followed by ~45 minutes of dry heaving. After bouts of dry heaving ended, instantly overheated and began sweating profusely (to the point I was as wet as if I had just gotten out of the shower). Eventually fell asleep again. Day after - headache, body aches, sweating alternating with chills. Feels like flu now (6:50 PM 17 April)

Patient was light headed, dizzy and stomach hurt. Patient then fainted. We offered to call the ambulance but patient and girlfriend refused. After questioning the patient we found out the patient hadn't eaten or drink water in the past 12 hours. we gave the patient water and snacks and some ice to cool him down. we also found out the patient has a fear of needles. once the patent started to eat and drink more he started to look and feel better.

The night of the shot, I began having symptoms: fever, shakiness, headache, nausea, neck pain, severe arm pain, and tiredness. My fever remained at 102 until the following morning when I began taking Tylenol every 6 hours for the fever. The tiredness, headache, neck pain, and arm pain lasted until 4/16/2021 (4 days).

Lightheaded 18hrs of the second vaccine, developed over the next 4 hours into a bad headache, minor nausea, light sensitivity. I think it may have been a fever but I couldn?t find a thermometer. I just remember feeling really hot.

pt complained of light-headedness, chest tightness, and nausea. symptoms lasted about20-30 minutes, then pt mostly complained of nausea, gave pt cold washcloth, water and meclizine 12.5mg. about 10-15 minutes after taking meclizine, said he felt good enough to walk out of store

Patient developed nausea and dizziness. He then had a witnessed syncopal event. HR was noted to be in the 50s. BP was noted to be 60 systolic initially. He denied any skin issues, mouth swelling, SOB or any other signs to suggest anaphylaxis. After several minutes, patient improved. Repeat BP 100/60, HR 56, SP02 99%. He recovered completely and elected to go home with family. Symptoms likely consistent with a vasovagal response to the injection.

Pt was sitting in post-vaccination area for his 15 minute observation time when he passed out after a few minutes. He had fallen onto the floor beside his chair. I immediately went out to pt. His eyes were fluttering. I shook pt and asked if he was okay. Pt opened his eyes and sat up seemed disoriented and said that he felt nauseated. I told pt to lay down on his back and elevate his feet on the chair, and told my technician to grab a bucket, an ice pack, and to call code white. We applied the ice pack to pts head. A member of management and an EMT who happened to be in the store reported over very shortly. Pt said he did think that he hit his head and his head did hurt. We asked the pt if he wanted us to call an ambulance. Pt said no, he was feeling a little better and he just wanted to wait a little bit and then finish his shopping. Pt was feeling better and was able to get back into the chair and sit. Pt appeared pale and we got him a drink of water. Pt sat and rested for sometime and after about an hour he said he was okay and was going to finish his shopping.

Pt report feeling clammy and "vision going like about to faint about 2 to 3 minutes after the shot. Pharmacist went over to check and found that patient is sweating. Patient also complained about nausea, hot and thirsty. Pharmacist did not find any rash or swelling on the face. Patient drank some water and did some slow, deep breathing. He reported feeling better after about 10 to 15 minutes. Pharmacy staffs called 911 when pharmacist first checking on the patient. EMR arrived in about 10 to 15 minutes and got blood pressure 125/76 heart rate 60 to 64. Patient said that he feels okay and EMR that he does not need to go to hospital. Patient agreed to check with MD and let pharmacy follow up next day.

Patient presented to Clinic with Allergic Reaction Symptoms, including shortness of breath, Chest tightness, and Nausea. Patient states he had gotten the shot roughly 1 hour before coming to Medical. Patient reported to Medical at 1038, where vitals were immediately taken. Patient Vitals were as follows: BP: 148/92 Temp: 98.1 Pulse: 90 R: 20 SPO2%: 100 EMS was called at 1052 and had arrived at 1102. EMS had left with patient at 1106. Patient did report a history of Childhood asthma, no allergies to medications.

Systemic: Dizziness / Lightheadness-Medium, Systemic: Flushed / Sweating-Medium, Systemic: Vomiting-Medium, Additional Details: Patient received first dose of moderna vaccine and within about 13 minutes he started vomiting and sweating. I had him wait for an additional 15 minutes seated until he felt well enough to leave. He refused me calling the ambulance. Someone came to pick him up. Called patient 30 mins later to check up and he said he was okay.

Health care worker reported that he had an adverse effect of the second dose of Moderna vaccine. He described normal side effects in the first two days following the second dose: fatigue, nausea, headache. Then noticed redness and a cluster of vesicles on the right upper forehead on 4/22/21. Diagnosed with Shingles and treated with antiviral medication.

Pain in arm at injection site started about 3 hours after injection. Full muscle pain and painful to lift or move arm for next 3 days. Dizziness/lightheaded starting around 7PM after moderate walk, then persisted for next 3 days. Fever of 99.5, strong chills and shaking, and mild nausea started at 10PM as I prepared for bed, persisted through the night, but chills and nausea gone in morning, and fever mildly subsided. Took NyQuil before going to bed to treat fever. Chills returned next night, but much milder than before and for shorter. Took ibuprofen, 2 tablets twice a day for next two days. Dizziness and mental fog persisted until 3 days after injection.

Experienced muscle pain, severe chills (3 hours), nausea and vomiting, and a high fever - I had contracted COVID-19 back in October 2020

Patient experienced nausea, dizziness, light headedness, sweating, ringing in the ears. I gave the patient an ice pack and he put it on his neck. Adverse events all resolved within 5 minutes of the ice pack.

Nausea and vomiting for 1 day. Prescription provided today for Zofran (4mg) 2 tablets by mouth 2 times a day as needed up to 4 doses

Severe flu-like symptoms. Woke up at 4am with a splitting headache, my ears ringing loudly, and my whole body aching like crazy. Severe dizziness any time I stood up. Slightly short of breath. High fever and chills. I?ve been taking ibuprofen every few hours and drinking lots of water, and I have felt better as time has gone on. Right now I?m still slightly achy but I don?t have a fever anymore and the dizziness and chills are gone. My appetite is starting to come back too.

The afternoon following the vaccine (5/7/21), he began to have a hard time breathing, at work, switching breathing between mouth and nose and felt warm. He is a dishwasher. By 5:00 pm, both of his arm muscles and stomach was cramping. He was having bigger problems breathing, and becoming woozy. The paramedics were called to his work, and he was transported to the ER as they felt he was having an allergic reaction. He had an IV, Tylenol for a fever, Atavan to relax, Benadryl for antihistamine as they could see his arms were red. The ER did urine and blood tests as well as a Covid test. He was also given oxygen to bring his oxygen levels back up

I was woken up by severe chest pain, which continued for the next few hours. I felt the pain above my stomach, below my sternum, and above my heart, and it was a sharp, stabbing pain. My heart was also beating faster than normal. The stabbing chest pain continued for the next few hours before eventually subsiding. However, for the next few days I occasionally felt a fleeting, stabbing chest pain. After a few days, it went away completely. I had never had such a sustained chest pain before.

Onset about 1 minute after vaccine, patient was dizzy, nauseated, cold sweats, and low blood pressure. After drinking some water, sitting for a few minutes, and eating some lunch, patient was feeling 100% better.

Pt received 2nd dose of moderna vaccine and within 1 minute started presenting symptoms of anxiety, sweat spells, and nausea. Pt was provided with water and a pharmacy bag in case of vomiting. Vomiting did not occur, only slight spit up into the bag. Pt remained conscious the entire time, no syncope occurred. Pt was observed for a time period of 30 minutes; pt felt much better after that time and was released from the pharmacy area.

My son suddenly came down with an unexplained internal infection that required hospitalization. The symptoms were nausea, high fever and chills. He was taken to the ER given antibiotics and kept overnight for observation. The doctor(s) could not determine the source of the infection. He recovered within 24 hours.

Headache from 6/20/21-6/24/21, Fever of 99.5-100.2, nausea, vomiting, chills, aches, arm still swollen around injection site. Symptoms were gone by 6/25/21

I started experiencing some sharp chest pain on the left side that started around a week or less after the shot. I?ve been stubborn and pushing it off (not currently insured so I didn?t want to get checked up). I?ve been experiencing the sharp pain on the left side of my chest off and on a few times a week at least. I?ve never experienced this but figured it would go away eventually. Last Sunday (July 11,2021) The chest pain was more severe than usual and I was very lightheaded and nausea and it didn?t get any better so I visited the ER. The EKG, chest x-rays, troponin levels, and other tests came back normal according to the physician assistant.

Patient started feeling dizzy, hot, and nauseated then his vision started to "black out." He vomited and his heart rate was elevated. He laid down on the floor for a few minutes. Then he sat down for about 20 minutes and he said he felt a lot better.

Patient received his first dose of the Moderna Covid vaccine and was advised to wait for 15 minutes after the vaccine for monitoring. Immediately after the vaccine, patient reported feeling light headed and blurred vision. He also stated afterwards that he had previously fainted after a blood draw. A certified pharmacy technician administered the vaccine and immediately notified the pharmacist once patient reported symptoms. Patient was given a bottle of water and instructed to remain seated and take deep breaths. He refused offer to call an ambulance. After about 5-7 minutes, patient reported feeling nauseous and vomited. Immediately after vomiting he reported feeling much better. He was instructed to remain seated and stay for observation for another 15 minutes. Patient remained stable and stated he was okay to leave. Followed up with patient in an hour and he reported feeling just fine.

The patient was administered the vaccination, observed for 15 minutes following the vaccination, after which he left to return to work at a pharmacy. The patient came back into the store about on hour later to pick up some things for his pharmacy and reported feeling a little chest pain. The patient was then informed to keep monitoring the pain and if it got any worse to go to the doctors office. About 2 hours later, the patient called when on the way to the emergency room and reported chest pain and a fever of 102 degrees (at the highest). Around 3pm, which was about 6 hours after the vaccination was administered, the patient reported chest pain was alleviated and that the emergency room treated him for a migraine/nausea with Tylenol #3, Fioricet and Zofran.

Injection site pain starting evening of 1/8/20. Mild swelling and severe pain at injection site morning of 1/9/20. Moderate progressing to severe headache, and intermittent chills all day; no fever. Reduced appetite. Mild muscle pain/stiffness.

Headaches and fatigue onset the night after the injection, and have persisted through today. The morning after receiving the injection, I had a migraine, nausea, fatigue, severe muscle aches, and fever/chills. All symptoms except headache and fatigue resolved by the next morning.

~3 hours after injection: chills, sweating, heart rate increase. 10 hours after injection: 102 degree fever, nausea, vomiting, chills, headache. 30 hours after injection: fever lowered to 100 degrees and nausea subsided.

I got the vaccine around 12:45 on a Monday - 1/4/21 4pm that day, started having a massive headache, laid in bed. Laid in bed all day, fell asleep around 10PM. 3:30AM woke up panicking with fever, tremors, chills, myalgia and very lethargic. Got out of bed to get tylenol and was shaking. Tuesday morning - woke up feeling the same as I did at 330 AM, plus no appetite on top of headache, chills, myalgia, lethargic and slept pretty much all day, felt fatigued. Wednesday was doing better, thought it was the end. Then suddenly about 7-8pm started having chills and shivering. I would say Thursday night is when I actually felt better and by Friday morning I was good with just residual ?brain fog/feeling dazed?

Adverse event started with nausea, chills, and body aches across the body. At 6:00 AM, I became very hungry, ate breakfast, and took two 500mg acetaminophen tablets which resulted in relief from body aches. I fell back asleep, woke up at 9:00 AM and was hungry again so I ate more food. I became hungry again at 12:00 PM and ate again. At 2:00 PM the body aches returned, I took two 500mg acetaminophen tablets and fell asleep until 3:30 PM. At 4:00 PM I became hungry again and ate. At 6:00 PM I ate another meal. At 9:00 PM I took two 500mg acetaminophen tablets and went to sleep. When I woke up the next day all symptoms had subsided.

Patient received his 2nd dose of the Moderna vaccine prior to hospital admission at unknown site. Patient reported generalized body aches and that his whole body hurts and feels likes he's "being poked with a knife." He endorsed associated symptoms of fever, nausea, back pain, and shortness of breath. The patient denies any sore throat, headache, cough, or vomiting. His back pain is at the tip of his shoulders, in the middle of his spine, and near his kidneys. The patient adds the abdominal pain is worst in the lower left quadrant. Patient admitted for bilateral lower extremity weakness limiting him to ambulate and to rule out Guillain-Barré. During admission, had fevers, photophobia, phonophobia, and nuchal rigidity. On physical examination patient was able to minimally wiggle toes, had no plantar/dorsiflexion, unable to move hip joints or knee joints bilaterally, lift his legs against gravity.

24hrs after vaccination severe myalgia, chills, headache, nausea, diarrhea, swollen lymph nodes, loss of appetite, and fatigue continued into 02/02/2021

Almost 12 hours post-injection, I experienced a rapid onset fever, soreness, muscle aches, dizziness, high blood pressure, headache. Upon awakening the following day (today, 20 hours post injection) severe allergies around eyes. 24 hours post injection, all day today, fatigue, severe dizziness, soreness, headache, high blood pressure, loss of appetite, and labored breathing.

Developed fever, chills, headache, nausea, fatigue, and severe pain at the injection sight. The fever developed about 8 hours after taking the vaccine. I had previously had COVID back in August and this is what it felt like for me.

Patient offered water and given protein granola bar to eat. Stayed with patient until symptoms subsided. VS reassessed and continues to be WNL. Patient voices no other needs or concerns. Patient left vaccine site @ 1507 ambulatory, alert, oriented x 3, and without medical concerns. 1446 VS: O2: 98% RA, R: 18, BP: 132/78, P: 67 1505 VS: O2: 100% RA, R: 18, BP: 122/70, P: 76

WE CALLED THE AMBULANCE AND THEY SAID IT COULD OF BEEN DUE TO LOW BLOOD SUGAR OR MAYBE HIS BLOOD PRESSURE DROPPED. WHEN THEY CHECKED HIS BLOOD PRESSURE HE HAD PERFECT BLOOD PRESSURE.

1st dose: sore arm, redness and swelling at injection site. 2nd dose: Fever for 36 hours, rose above 104 for several hours, diarrhea, sweating, pain at injection site, lighted headed, headache, difficulty breathing,

Member presented with profuse sweating and said he did not feel well at all. Because I do not have aassigned to the clinic, I went to ask an to do a physical examination on the member. The went retrieved water for the member to help him cool down and another patient was in need of my assistance. As I returned I saw the conducting his physical examination of the patient where he started to examine the member where the member did not present with any hives on his body but his tongue was slightly larger than normal and he started to clinch his stomach and he could not physically drink the water and he then fell over gasping for air and eyes bulging(you could hear him

The patient presents with 25-year-old male with no significant medical history presents to the emergency department for evaluation of nausea, vomiting, generalized body aches, headache 1 day after his Covid vaccination. Reports associated subjective fever and chills. No diarrhea. No surgeries. He has tried no medication pain. He has had 3 episodes of nonbilious nonbloody emesis.. The onset was 6 days ago. The course/duration of symptoms is constant. Location: generalized. The character of symptoms is Malaise, nausea, vomiting. The degree at onset was minimal. The degree at present is moderate. Risk factors consist of Recent COVID-19 vaccination. Therapy today: none. Associated symptoms: nausea, vomiting, chills, headache and Generalized malaise Reexamination/ Reevaluation Re-examination/Re-evaluation: Patient is significant symptoms after Toradol, IV fluids, Zofran. He was able to tolerate p.o. intake. Laboratory evaluation was unremarkable. UA showed likely contaminant. Pt has no urinary symptoms. Discussed postvaccination syndrome with the patient. Strict return precautions discussed. Patient expressed understanding and agreement treatment plan. Will discharge home with a prescription for Zofran to be taken as needed for vomiting. NSAIDs and Tylenol as needed for generalized pains. Follow with primary care provider in 3 to 5 days.

At 246pm - Pt alerted the nurse he was felling nauseous. Pt was assisted to wheelchair and to private area.Pt was pale and diaphoretic, pt vomited 3 times. 252 pm BP 100/80 HR 75 02- 100% Resp 20 Pt states he feels much better after vomiting. 256pm- Pt color and skin returned to WNL after vomiting. BP 110/60 Hr 68 resp 18 02 100% 303pm- BP 110/72 Hr 58 02 99% Resp 16 Pt states he smoked marijuana and ate food prior to arrival , and that he had similar nausea after first dose but did not vomit the first time because he did not have anything to vomit. Pt states he also has hx of social anxiety. 308pm- O2 - 99% resp 16 HR 68 Pt mother in parking lot to transport pt home. Pt states he feels better and is ready to go home. Pt feels good upon standing and standing VSS - BP 110/77 HR 68 02 sat 99% resp 18 Reviewed discharge instructions with pt and to call emergency responders or go to nearest ED if symptoms worsen. Pt was provided vsafe flyer. Steady gait exiting clinic.

First dose: some arm soreness and little pain during this time Second dose: did not start off that bad, felt pain when moving arm around (lifting arm) around 3-4pm - felt feverish (about 12 hours after dose), couldn't sleep, felt chills and that lasted until next day - few hours after injection until the next day, felt generalized exhaustion, muscle aches and fatigue - few hours after injection felt nauseous and it went away after waking up from night sleep the next day - took ibuprofen 800mg around the time of injection and then took another ibuprofen 800mg in the evening

Started to experiment COVID-19 like symptoms the next morning after I received the vaccine. The symptoms included headache, nausea, muscle aches, joint aches, fatigue, fever, and chills.

Approximately 2 minutes after COVID-19 Moderna vaccination 0.5ml injected, patient fainted in the chair located in our 15 minute waiting area. RPh was called to respond. Patient was unresponsive upon arrival. Pulse present and strong, breathing appeared normal. Patient was aroused by touch and voice. Upon arousal patient was lethargic but could indicate name and DOB. Patient verbalized feeling warm, faint, and nauseated. Care was administered by visual monitor, cold compress to neck and forehead, water, and fan placed near patient. Patient's girlfriend was present and was a diabetic and took patient's BS reading with her own glucometer. BS was 98. Patient vomited x 1. Took approximately 50 minutes for patient to be fully awake and ready to try standing. Upon standing patient was steady on the feet. Patient was advised to discuss reaction with primary care provider and was assisted, when ready, to the door.

Developed headache, tachycardia, painful heartbeat / strong feeling of heart beat in upper chest/throat, moderate nausea, sore injection site. Symptoms cleared after 48 hours, except for sore injection site, which cleared after 72 hours. Moderate nausea was treated with Zofran ODT during the first 48 hours of symptoms, prescription from urgent care.

I had an anxiety attack. The physical symptoms were a severe headache, fever, vomiting, shaking, and aching body.

Patient reported feeling unwell shortly after receiving the covid 19 Moderna vaccine. He was seated in a chair and had flushing, seating, dizziness, and nausea. The paramedics we called to assess the situation. We gave him a bottle of water and monitored him for approximately 10 minutes until the paramedics arrived. He was assessed for blood pressure and vital signs. The Paramedics determined he was ok to leave on his own after he felt able. he remained in the waiting area for approximately another 10 minutes then left.

About 5-10 minutes after the shot was administered, patient complained of some dizziness, nausea, lightheadedness. Few minutes later. some tingling of the lips. Patient was given about 50mg of diphenhydramine about 10 minutes after the shot, some water to drink and his blood pressure was checked. BP was 114/78 and heart rate was 68. Patient was allowed to sit for about an hour and felt better over that time and was able to go back to work cautiously

Redness/swelling of arm 24 hours after shot. Fatigue/tiredness next morning (12 hours after). Flu-like symptoms (nausea headache) and body aches next afternoon (24 hours after).

Within two hours, I started feeling slightly nauseous and lightheaded. Overnight, I woke up with a 100.5 degree fever. I had chills at the time and a sore throat. By midmorning the next day the nausea and fever went away. I felt very fatigued the whole next day. By the second day after the vaccine I felt fine.

11:43 am Patient received first vaccine of COVID-19. 11:53 am Patient had initial syncope and pharmacist went to do a quick check of systems and patient was alert and responsive. Patient claimed of having upset stomach. Patient was left at chair. Patient then left to go to the restroom with father. Pharmacist went to find patient and went to the restroom as well. 11:58 am Pharmacist found patient and patient was being supported and held up by father. Pharmacist had patient lay down and elevated feet. Pharmacist called EMS to make sure patient was cleared and okay. 12:05 EMS arrived and did a initial work up and found that the patient's heart rate, blood pressure and vitals appeared normal. Patient claimed that he felt fine and normal. EMS asked if father, who was with patient, would be able to keep him home and down and relaxing for the day.

Nonstop vomiting and diarrhea. Went to ER and was told I had very high glucose and lactic acid. I have no history of diabetes.

I woke up around 6am with a terrible headache, body aches and nausea. Around 3pm, I began experiencing a fever of 101.3°F and chills. The fever, chills, headache, and nausea have subsided, but body aches remain. I am also still experiencing what I believe to be swollen lymph nodes around my collar bone.

Per client history of asthma 10years ago, no medications, no known allergies. Vitals 1645: blood pressure 141/91, pulse 66, oxygen 99%. Per client feeling nauseous and blurry vision. EMT gave emesis bag and water. Vitals 1650: blood pressure 138/87, pulse 62, oxygen 100%. Client stated nausea resolved. At 1653 client stated headache worsened. At 1700 client stated vision improving. Vitals: blood pressure 138/90, pulse 75, oxygen 100%. Per client all symptoms had resolved. Client left facility with steady gait at 1735 after completing 1hr observation. At 1746 client returned to EMT station, stated once he began to drive he felt nauseous again. Vitals at 1750: blood pressure 148/90, pulse 65, oxygen 98%. Vitals at 1756: blood pressure 129/92, pulse 70, oxygen 98%. Per client nausea about the same. Vitals 1800: blood pressure 133/95, pulse 64, oxygen 98%. Per client nausea slightly improved, light headed, and slight vision changes per client. RN Thong offered client paramedics for further assessment, client denied, would like to stay longer in observation. Vitals at 1809: blood pressure 140/92, oxygen 72, oxygen 99%. Per client feeling hungry, still nauseous. 1820 vitals: blood pressure 136/98, pulse 66, oxygen 96%. Nausea and vision improving. 1831 vitals: blood pressure 134/87, pulse 69, oxygen 98%. Per client vision same, nausea improving. Client had family member come and pick him up. RN advised client to follow up with provider regarding reaction. ER precautions also given. Client left facility with steady gait at 1840.

PT COULD NOT HEAR OR SEE AFTER VACCINE . HIS BLOOD PRESSURE WAS DROPPED . i WAS CHECKING HIS BLOOD PRESSURE EVERY 5 MINS. H E WAS REALLY HUNGRY SO GAVE HIM SOME CRAKERS AND WATER. CHECKED AGAIN HIS BLOOD PRESSURE AND AFTER THAT HE WAS FINE . WE HAD MONITOR HIM FOR 30 MINS .

The day after the first dose my Crohn's symptoms flared up, I had diarrhea cramping and abdominal pain. My Crohn's had been under control for a year before the vaccine. It was persistent for two weeks. I was prescribed a muscle relaxer by my doctor and I used Tylenol for the abdominal pain. The flare up was a little less severe than my usual flare up and it settled back down after my next infusion at three weeks after the vaccine. I had the second dose I had a low grade fever for a few hours and no GI issues. Second Dose Lot 028L20A Date 01/30/2021.

Patient came for second dose of Moderna vaccine and upon reviewing PQCF the patient indicated that for his first dose during the 30 mins observation period he experienced difficulty swallowing and dizziness. The patient has a history of anaphylaxis with sulfa drugs. Patient did not inform of adverse event to his PCP nor the staff of the pharmacy. He was very nervous. He was advised to consult with his PCP the risk vs benefit of receiving his second dose and to inform us of the final decision for documentation. Second dose of vaccine was not administered until further communication between patient and PCP.

24 hours after the second Moderna dose, I began to feel very weak with a strong headache and a low grade fever. By Thursday (48 hours after vaccination) the headache was very strong and by that night it was debilitating. The fever had increased as well to between 101-102 degrees. Thursday night Friday I treated symptoms with alternating doses of Ibuprofen and Acetaminophen, which succeeded in reducing the fever but did not rid of the headache. Saturday the debilitating headache persisted with extreme light sensitivity and nausea. The pain became so great Saturday night that my wife took me to the Emergency Room. There they performed a brain scan with an MRI that came back normal and treated the pain with Morphine. I was sent home around 8am Sunday morning with a prescription for Toradol. The pain did not decrease with the Toradol and by 6pm Sunday I was vomiting blood and the pain was excruciating. At the advice of my primary care doctor, I went back to the Emergency Room. In the Emergency department they performed a Lumbar Puncture and collected spinal fluid for testing, as well as blood for lab work. The pain was managed with Fentanyl. Late that evening the Hospital admitted me. I was given two more MRI's of the brain - one with contrast. I also underwent an upper scope of my esophagus, stomach, and upper colon which found 3 small ulcers likely due to the high volume of NSAID pain relievers taken over the previous days. The lumbar puncture revealed that my white blood cell count was 51 and they officially diagnosed aseptic meningitis.

First night after receiving 2nd dose of the Moderna Vaccine, I experienced a high fever and chills, sweating and a disturbing Ringing in my head. The second day after was roughly the same as the first night. I expected some of these symptoms and was willing to power through them. It is now past 2 weeks since I have had the Vaccine. I still have a maddening Ringing in my head. Different than a Ringing in my ears. It does not feel the same. I feel pressure in my head, resulting in constant headaches that never subside. I have felt nauseous at random times, stomach pain from not being able to eat, and I am for the first time in my life experiencing anxiety and depression. I feel like my pain will never go away and I am scared. This pressure/pain in my head accompanied with the constant ringing is driving me insane. I have lost time at work, lost time with friends and family trying to sleep off my pain and symptoms but it never helps. I have taken headache medication which helps a small amount for the head pain, but leaves me with an upset stomach. I can not take these pain pills any more. My Doctor, had me do a blood test and we are awaiting results as of today 5/25/21. I do not think they will find anything that will be able to help me with what I feel this Vaccine has done to me. I do not know what to do besides try not to cry myself to sleep anymore. I hate this and I do not want to feel this way anymore.

Pt said he was disoriented , lightheaded and dizzy. We had him sit to observe him. He was flushed all over body and cold to touch. He stated he was feeling nauseous. I led him to a private space and had him lie down. He stated that his lips were tingling. I gave him 25mg liquid diphenhydramine and continued to observe. He said throat was very dry and feeling worse. I contacted 911 and they came to the clinic to evaluate the patient. They checked blood pressure, blood sugar, heart rate and rhythm. At 5:25 he decided he was feeling a little better and he refused transport by the ambulance. We arranged for an emergency contact to come pick him up.

1: He had his second Moderna vaccine shot at noon May 12,2021. Within one hour, he consistently had hiccups, then followed vomiting. It last for 24 hours without any food and sleeping. 2: 5/13/2021, he was sent to Urgent care, had infusion, and Ondansetron. But he constantly had hiccups and vomiting. He could not fall asleep due to the illness.3: 5/17/2021, his vomited dark brown color material, and his defection had black color. He was sent to urgent care second time, but transferred to ER at hospital.4: 5/24/21, he started hiccups and vomiting again. His 5/25 blood tests had significant problems in liver, kidney and pancreas ( details in blood test reports). These test results make us very anxious.

Systemic: Fainting / Unresponsive-Mild, Systemic: Flushed / Sweating-Severe, Systemic: Nausea-Medium, Additional Details: Extreme sweating, nausea & short fainting spell. Gave patient water and sugary snack. Mentioned he may not have eaten enough that morning

Patient compliaing of lightheadedness and headache after receiving the 2nd COVID vaccine Moderna at 13:12. Two EMT's reported to Hospital on site of similar reaction to 1st dose of Moderna. At 13:32 Client complains of lightheadedness and headache, after 20 minutes in observation. Clients vitals blood pressure: 144/86, heart rate: 79, respirations: 14. oxygen saturation 97%. Client mentions history of lightheadedness nausea and headache during his first Moderna vaccine. Client is alert and oriented to person, place, time and date. Client denies nausea, chest pain, abdominal pain, shortness of breath, or any noticeable rashes or itchiness. At 13:38 Client states headache worsens rating at a 6/10 pain scale, and lightheadedness, Client states "head feels fuzzy" and had breakfast at 1100am. Clients vitals blood pressure: blood pressure: 131/86, heart rate: 70, respirations: 14. oxygen saturation 98%. Client was given water and juice and had client move to anti-gravity chair. At 13:44 Client states headache continues to be a 6/10 and "comes in and out", clients vitals blood pressure: blood pressure: 142/87, heart rate: 64, respirations: 14. oxygen saturation 98%. At 13:54 Client stated headache worsens to 7/10 on pain scale and still feels lightheadedness. Client stated that he has a driver that will take him home. Clients vitals blood pressure: blood pressure: 131/80, heart rate: 66, respirations: 14. oxygen saturation 97%. PHN on site advised client to contact emergency medical services or follow up with primary care provider if conditions worsen. Client agreed and verbalized understanding about information provided. At 1402, clients vitals blood pressure: blood pressure: 127/85, heart rate: 66, respirations: 14. oxygen saturation 97%. Client was escorted to parking lot with EMT at 14:08 with a steady gait.

Patient was walking in waiting room, passed out, fell backwards and hit his head on a chair. When I came to the waiting room patient was actively seizing and bleeding from a laceration on the posterior part of his scalp. Patient moved from under chair and placed in recovery position. Active seizure time from my arrival was 30 seconds. Patient was minimally responsive and postictal for approximately 5 minutes and diaphoretic. Vital signs taken and oxygen placed on patient. Color returned to normal, and VSS. BS was 108 on glucometer. EMS called and patient transported out of facility. Patient is a 35 yo M with no sig pmhx presenting after syncopal event after receiving his COVID vaccine. Fell from standing, denies dizziness, SOB, CP, or other symptoms prior to the event. He hit the back of his head when he fell. He presented to the ED, at which time he felt very nauseous and per notes he was confused and perseverating. Admitted to the SICU for R frontal IPH and R frontal SDH.

sudden onset hour and a half prior to arrival chills, nausea, shortness of breath, sharp stabbing global chest pain with cough only nonproductive, tachycardia 120 bpm, subjective fever chills after having received moderna vaccine 3 days ago. Tachycardic to 120, active chest pain CTA negative

Dizziness, nausea, chills lasting 20-30 minutes tightness in chest / light pain near heart, lump/swelling in throat lasting 5-6 hours then tapering down for the next 4 hours

nausea and diarrhea after 1st dose. 100.6 fever, and reports feeling like intestines were twisting up.

Sharp shooting heart discomfort, sharp heart irritation, heart burn, feeing of low fever, internal flush, internal warmth, wake in middle of sleep in need of sitting up on chair, intense feeling of imminent heart attack, intense feeling of imminent heartbeat stopping

After 2-3 minutes given the shot, patient reports feeling ill, nauseous, dizzy and sweaty all over. He denies difficulty of breathing, hyperventilation, or rashes. He asked to use the bathroom facility in case if he vomit. He did not vomit. He splashed water onto face and reported that made him felt better. He rested on a sofa in the pharmacy and was given a cup of water. After 3-5 min, he reports all symptoms have subsided and thought maybe he was too anxious due to anticipation of severe reactions after the second shot. Patient continues to deny trouble of breathing nor rashes throughout the 20 minutes resting in the wait area. Benadryl & Epi-Pen were not needed nor given.

Patient reported being hospitalized day after vaccination: symptoms included severe vomiting, chills, fever, fatigue. Could not keep food down during stay. Given IV medications for vomiting.

Syncope. Patient experienced Nausea, sweating, pain, and dizziness. Patient was told to sit down, given and ice pack, and water

Pt was administered Moderna on 07/14/2021. Pt called on 7/20/2021, left a Voice Mail stating he would like a call back to discuss side effects to the vaccine. I called patient back the morning of 07/21/2021, the patient states to me that two hours after the vaccine was administered he started to shake uncontrollably and was very cold, then he blacked out (Patient states he may have had a seizure.) Pt woke up outside behind his vehicle in a pool of his own vomit. I asked if 911 was called and the patient said no because he could not move or walk. The patient also stated that he felt bad all day, Thursday (07/15/2021), the day after the vaccine was administered, woke up on Friday (07/16/2021) feeling relatively normal still feels the urge to vomit when eating certain foods.

In addition to CDC expected symptoms (swelling of the face, tiredness, headache, muscle pain, chills) I experienced nausea, coughing, sore throat, vomiting or gaging after food or coffee.

Pt reported not feeling well. He looked pale, sweaty and appeared to be breathing normally. He reported not having difficulty breathing but was feeling light headed and thought he may vomit. Monitored patients blood pressure and respiratory rate--both were normal (114/76; 93bpm; 19rpm). Called ambulance who arrived in about 7 minutes and took over his care. He was still not feeling well and was light headed/dizzy and feeling like he could throw up.
