Patient received vaccine that she should not have. she was questioned again at the check in station that she goes on calls and interacts with people on the call. She answered yes to both and said she was sent by her Chief. The borough supervisor was called that same day and told no one but first responders dealing with the public were to be sent to the clinic, he called back and said he had e-mailed the police and fire departments and they had given a revised count that eliminated all of those not eligible. When her shot was entered in the PA-siis data system. The correct birthday was changed on the online form and she was denied for any further vaccinations. She then clarified that she was 14 and thought she had to get it because her Chief sent out an email indicating that active fire fighters were to get vaccinated if they wanted and she thought she should. I then instructed her to bring her parents or guardians into the clinic. Her grandfather; her guardian spoke to us and said he thought she was supposed to get it. I had her sit for 30 minutes with no problems. I followed up the next morning and she indicated feeling fine, I then reviewed all the symptoms that could occur and asked them to call me on my cell or if severe seek medical attention. I have followed up daily she has reported sore arm and a headache at work that lasted 30 minutes on1/26/2021, A mild itch at the injection sight 1/27/2021 but gone in the morning. We will continue to follow up daily with the vaccine

administration error only, no side effects Moderna was inadvertently given by Nurse instead of Pfizer and the child is only 14 years of age she was educated about the vaccine that was given, provided materials, kept for monitoring for 30 minutes. provided with public health nurse contact info. no adverse reaction observed, reported, or noted aside from minor redness at injection site after 30 minutes. child was released home with grandmother who is her legal guardian. Contacted department of health who advised us to complete this vaers report but to go ahead and have them come back for her second dose of moderna after 28 days

Bruised arm; Itchy; Accidental administration of the Moderna COVID-19 Vaccine to a patient who is 15 years old; A spontaneous report ( was received from a pharmacist concerning a 15-year-old, female patient to who accidently received Moderna's COVID-19 vaccine (mRNA-1273) and experienced accidental administration of the Moderna COVID-19 vaccine to a patient who is 15 years old/product administered to patient of inappropriate age, itchy/pruritus and bruised arm/contusion. The patient's medical history was not provided. No concomitant product use was reported. On 25 Jan 2021, the patient received their first of two planned doses of mRNA-1273 (Lot number: 012L29A) in the right arm intramuscularly for prophylaxis of COVID-19 infection. On 25 Jan 2021, a 15-year-old female patient received an accidental dose of the Moderna COVID-19 vaccine and experienced an itchy arm. On 28 Jan 2021, the reporter noted the patient had a bruised arm. Treatment information was not provided. Action taken with mRNA-1273 in response to the events was unknown. The outcome of the event, accidental administration of the Moderna COVID-19 Vaccine to a patient who is 15 years old/product administered to patient of inappropriate age, was considered resolved on 25 Jan 2021. Patient is 15 years old and received the first dose of the Moderna vaccine. The Moderna vaccine is authorized for people age 18 and older. According to the patient's mother, they did not realize this until after vaccination. Patient was advised not to receive the second dose until 18 years of age. Patient reported redness and swelling at the injection site. No other adverse effects reported.
